米力农治疗新生儿持续肺动脉高压临床观察

摘　　要：目的探讨米力农对新生儿持续肺动脉高压(PPHN)的治疗效果。方法 35例确诊新生儿持续肺动脉高压的患儿应用米力农以0.5μg·kg-1·min-1速度持续静脉滴注在治疗中根据血压变化适当增加或减少米力农输入速度,连用3天,于治疗前、治疗后动态监测患儿血压、肺动脉收缩压(PASP)、经皮氧饱和度(SpO2)及动脉PaO2等变化。结果应用米力农治疗后患儿临床症状明显改善,肺动脉收缩压明显下降,血氧饱和度、动脉氧分压明显升高,差异有显着性(P<0.01),35例接受米力农治疗的PPHN患儿治愈22例,好转8例,无效5例,有效率85.7%。结论米力农对新生儿持续肺动脉高压具有良好治疗效应,可有效降低肺动脉压力,改善临床症状。Objective To explore the effect of milrinone for the treatment of persistent pulmonary hypertension of the newborn（PPHN）.Methods 35 newborns with PPH were treated with intravenous drop infusion of milrinone at 0.5 μg·kg-1·min-1 for three successive days,during the process the infusion rate of milrinone was adjusted in accordance with the change of blood pressure.Blood pressure,pulmonary artery systolic pressure（PASP）,SpO2 and PaO2 were measured before and after the treatment.Results After treatment with milrinone,the clinical manifestations of infants were significantly improved,PASP notably decreased,SpO2 and PaO2 obviously increased,there was significant difference（P0.01）.Of the 35 PPHN,22 cases were completely cured,the symptoms of 8 cases were significantly improved,no effect in 5 cases,the effective rate was 85.7%.Conclusion Milrinone is effective in treating PPHN,which can decrease PAP and improve clinical manifestations.
