电视胸腔镜二尖瓣置换手术体会

摘　　要：目的总结电视胸腔镜二尖瓣置换手术体会。方法采用右股动脉插供血管,右股静脉和(或)上腔静脉插引血管建立体外循环。右腋下打孔长阻闭钳阻闭升主动脉,心脏停跳顺行灌注保护心肌,在电视胸腔镜下胸壁打三孔完成二尖瓣置换术13例。结果升主动脉阻闭时间45～125min,平均65min;手术时间3.6～5.8h,平均4.2h。全组无死亡。结论电视胸腔镜下行二尖瓣置换创伤小,美容效果好,是可行的、安全的。Objective To summarize experience in mitral valve replacement by video-assisted thoracoscopic surgery.Methods Applying right femoral arterial catheterization for blood supply,right femoral vena and（or）superior vena catheterization as blood drainage to establish peripheral extracorporeal circulation.Accessed a hole in the right chest wall for clamping ascending aorta,myocardial protection with antegrade perfusion and accomplished mitral valve replacement for 13 patients by video-assisted thoracoscopic surgery with three-hole access in chest.Results The clamped time was 45~125min,average 65min;The operation time was 3.6~5.8h,average 4.2h.There was no death.Conclusion Video-assisted thoracoscopic surgery for mitral valve replacement caused minimum invasion,with good cosmetology effect.It is a feasible and safe method.
