摘 要:目的探讨灰阶超声与超声弹性成像应变率值在甲状腺结节良恶性鉴别中的应用价值。方法对124个甲状腺结节进行灰阶超声与超声弹性成像检查,测定弹性应变率值,与病理结果比较分析灰阶超声、弹性应变率值对甲状腺结节良恶性的诊断价值。结果灰阶超声各项参数及参数的组合、弹性成像应变率值在甲状腺结节良恶性组间差异均有统计学意义(P<0.05);比较各种方法的ROC曲线下面积,发现弹性成像应变率值在鉴别甲状腺结节良、恶性方面准确性更高;根据ROC曲线,约登指数最大的切点0.713对应的应变率值最佳诊断界点为5.6。结论超声弹性成像应变率值有助于甲状腺结节良恶性的鉴别,准确性优于灰阶超声。Objective To investigate the value of ultrasonic elastography and gray scale ultrasound in diagnosing thyroid diseases. Methods Gray scale ultrasound and elasticity imaging with strain ratio were used to examine 124 thyroid nodules. The gray scale ultrasound parameters and strain ratio in elasticity imaging were retrospectively reviewed and compared with pathology. Results The parameters in gray scale ultrasound and their combinations, strain ratio in elasticity imaging were statistically different between benign and malignant lesions (P〈0.05) . Compared AUC (area under the curve) of different ROC, the diagnostic accuracy of strain ratio was superior to that of parameters in gray scale ultrasound and their combinations. According to ROC analysis, the cut off point of strain ratio was determined as 5.6 and Youden's index was 0. 713. Conclusions Strain ratio could he used to differentiate benign and malignant thy roid nodules, and the diagnostic accuracy was superior than gray scale.
