实时三维超声心动图对房间隔缺损患者左室收缩功能的评价

摘　　要：目的应用实时三维超声心动图(RT-3DE)研究房间隔缺损(ASD)患者手术前后左心室容积及收缩功能的变化。方法采集59例ASD患者(其中封堵术28例,修补术31例)及33例正常对照者的左心室全容积成像,测量左心室容积及射血分数,比较手术前后左心室收缩功能的变化。结果 ASD组术前左心室舒张末期容积(LVEDV)、左心室每搏输出量(LVSV)及左心室射血分数(LVEF)降低(P<0.01),左心室收缩末期容积(LVESV)与对照组无明显差异(P>0.05);术后ASD封堵组与对照组比较无统计学差异(P>0.05),修补组较术前改善但仍低于正常对照组(P<0.05)。结论 ASD患者术前左心室容积及收缩功能降低,术后封堵组与正常对照组无差异,修补组较术前改善但仍降低。Objective To investigate left ventricular systolic function before and after operation in atriel septal defect （ASD） patients using real-time three-dimensional echocardiography （RT-3DE） . Methods Left ventricular full volume imaging of fifty-nine ASD patients （the cardiac implant closure group with twenty-eight patients and the surgery group with thirty-one patients） and thirty three normal persons were acquired by RT-3DE. Left ventricular volume and ejection fraction were calculated. Left ventricular systolic function after operation in ASD patients were compared with those before operation. Results Left ventricular end-diastolic volume （LVEDV）, left ventrieular stroke volume （LVSV）, left ventricular ejection fraction （LVEF） in ASD patients decreased （P〈0.01）, and left ventricular end-systolic volume （LVESV） had no significant difference （P〉0.05） compared with those in the normal group. After operation, LVEDV, LVESV, LVSV and LVEF had no significant difference between the cardiac implant closure group and the normal group （P 〉 0.05）, while those became better but still decreased in surgery group （P%0.05） . Conclusions Left ventrieular systolic function （LVSF） and volume in ASD patients decreased, and after operation there is no difference between cardiac implant closure group and the normal group, the LVSF in surgery group had became better but still decreased.
