胰十二指肠切除术的围手术期治疗探讨

摘　　要：目的探索胰十二指肠切除术围手术期治疗。方法采用回顾性分析方法,对我院2008年1月至2010年2月期间3例行胰十二指肠切除术患者的临床资料进行分析,特别是术前诊疗、术中处理及术后治疗的探索,以提高手术治愈率,减少手术风险,降低并发症。结果 3例痊愈出院,其中1例腹部切口感染。结论胰十二指肠切除术围手术期治疗重点在于手术操作,各吻合口的处理是防治瘘的关键,手术后给以抗炎、抗酸、抑酶,纠正酸电解质平衡紊乱,营养支持,纠正低蛋白和手术后失血及补充各种维生素。Objective To explore treatments of perioperative period in pancreaticoduodenectomy.Methods Retrospective method was performed to analyze 3cases of pancreaticoduodenectomy that take pereoperation,operation and postoperation treatment from may Jan.2008 to Feb.2010 in our hospital and to rais successful rate of operation,reduce the risk of operation and lower complication of operation.Results They were healed out-hospital,which 1 case was wound infection.Conclusions The keys of treatment in pancreaticoduodenectomy was the high technical operators and re-beuiding of three anastomotic in operation and was taken measures in postoperation which were controlling infection,injecting acid-suppressing agents and trypsin inhibitor,keeping the equilibrium of water,electrolytes and acids and bases,injecting total parenteral nutrition and preteot,blood and multi-vitamins.
