摘 要:目的探讨术前束腹胸式呼吸训练对老年患者腹部手术后肺部并发症的预防作用。方法将86例腹部手术老年患者随机分为实验组和对照组,实验组45例行术前束腹胸式呼吸训练,对照组41例行术前常规呼吸训练,分别于呼吸训练前和呼吸训练1周后检测束腹状态下肺功能(IC、FEV1.0、FEV1.0/FVC)的变化,并观察和分析术后两组患者肺部并发症的差异。结果呼吸训练前两组患者束缚状态下肺功能差异无统计学意义(P>0.05),呼吸训练1周后两组患者束腹状态下肺功能差异有统计学意义(P<0.01),并且实验组术后肺部并发症显著少于对照组(P<0.01)。结论术前束腹胸式呼吸训练能显著改善老年患者的肺功能,并能预防腹部手术后肺部并发症。Objective To study the prevention of respiratory complications after abdominal surgery in elderly patients by pre- operative abdominal binding thoracic breathing exercise. Methods Eighty - six elderly patients scheduled for abdominal surgery were randomly divided into trial group and control group. The trial ( n = 45 ) exercised the pulmonary functions with the abdomen bound and the eoutrol group (n =41 ) without it. The pulmonary functions ( IC, FEV1.0, FEV1.0/FVC) were tested without abdominal binding and breathing exercise. Then breathing exercise was done for one week, and the pulmonary functions under abdominal binding were tested before and a week after the exercise respectively. Respiratory complications were compared between the two groups. Results There was no significant difference in pulmonary function before breathing exercise( P 〉0.05 )but significant difference was observed on the 7th day of the exereise(P 〈0.01 )with the abdomen bound. Besides, there was significant difference in respiratory complications(P 〈 0.01 ) between the two groups. Conclusions Preoperative abdominal-binding thoracic breathing exercise can improve the preoperative pulmomary function in elderly patients and prevent postoperative respiratory complications.
