摘 要:目的研究急性百草枯中毒(acute paraquat poisoning,APP)患者的肝、肾功能各项指标水平变化的临床意义,以探讨百草枯中毒患者肝、肾脏损伤的发病机制。方法 APP患者49例(A组),根据患者服毒量和临床体征,分为轻度(A1组)、中度(A2组)和重度(A3组)中毒组,48h内检测各项肝、肾功能指标。另随机选择49例健康体检者为对照组(B组)。结果 A组与B组比较:肝功能指标(ALT、AST、TP、ALB、TBIL、DBIL)及肾功能指标(BUN、Cr、UA)两组间的差异均有统计学意义(P<0.05)。A1组与B组比较:除TBIL、DBIL两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A2组、A3组与B组比较:肝功能指标(ALT、AST、TP、ALB、TBIL、DBIL)及肾功能指标(BUN、Cr、UA)两组间的差异均有统计学意义(P<0.05)。A1组与A2组比较:除UA两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A1组与A3组比较:除UA两组间差异无统计学意义外(P>0.05),其余指标均有统计学意义(P<0.05);A2组与A3组比较:除ALT两组间差异有统计学意义外(P<0.05),其余指标间差异均无统计学意义(P>0.05)。结论百草枯中毒患者的肝、肾可有不同程度的损伤,并随中毒程度的加重而加重。Objective To study the clinical significance of indices of the hepatic function and renal function in acute paraquat poisoning( APP), and the pathogenesis of hepatic and renal damage. Methods Forty - nine patients diagnosed with acute paraquat poisoning (group A)were divided into three groups :the mild poisoning group( group A1 ) , the moderate poisoning group (group A2 ) and the severe poisoning group( group A3 ) according to the amount of poison taken and clinical symptoms . Forty -nine healthy outpatients undergoing medical check - up ( group B) were randomly selected as the control group . The clinic indices of their hepatic function and renal function were determined in 48h,including ALT ,AST ,TP ,ALB ,TBIL ,DBIL ,and BUN ,Cr ,UA. Results Compared with group B, all determined indices in group A showed statistically significant difference(P 〈0.05) , so did those in group A1 (P 〈 0.05 ) except for TBIL and DBIL( P 〉 0.05 ). Compared with group B: all determined indices in group A2 and A3 showed statistically significant difference(P 〈0.05 ). Compared with group A2 and A3, all indices in group A1 showed statistically significant difference ( P 〈 0.05 ) except UA ( P 〉 0.05 ). Compared with group A3, none of the indices in group A2 showed statistically significant difference (P 〉 0.05 ) except ALT( P 〈 0.05 ). Conclusions The hepatic and renal function in the APP patients are damaged to different degrees. The more severe poisoning is , the heavier the damage.
