肾病方治疗难治性肾病综合征疗效观察

摘　　要：目的:观察肾病方治疗难治性肾病综合征的临床疗效。方法:选择68例难治性肾病综合征的患者,采用随机表随机平均分成2组,对照组给予糖皮质激素和环磷酰胺并对症治疗,治疗组在对照组治疗方案的基础上给予肾病方(基本方为黄芪、党参、茯苓、山药、芡实、金樱子、菟丝子、山茱萸、益母草、川芎、丹参、地龙、水蛭)治疗,2组疗程均为56d,观察2组24h尿蛋白定量、血浆白蛋白浓度、血液粘滞度、血脂及中医症候积分变化。结果:治疗组和对照组均可降低24h尿蛋白定量,提高血浆白蛋白水平,改善血液粘滞度、降低血脂,但两组间比较,治疗组上述实验室指标变化及中医证候积分改善更显著(P<0.05)。结论:肾病方能提高难治性肾病综合征的疗效。Objective：To determine the efficacy of Shenbing Fang in the treatment of refractory nephrotic syndrome.Methods：68 patients with refractory nephrotic syndrome were randomly assigned to Shenbing Fang group or western medication group.Western medication group received glucocorticoids,cytotoxic drugs and symptomatic treatment.Shenbing Fang group received western medications and Shenbing Fang.The treatment duration in both two groups was eight weeks.Outcome measures included the changes in 24 h urinary protein excretion,plasma-albumin concentration,serum lipids,blood visclsity and accumulated scores of TCM syndrome.Results：In both two groups the 24 h urinary protein excretion and serum lipids decreased,the plasma-albumin level increased and the blood visclsity was ameliarated.However,the Shenbing Fang group showed significant difference in the change of the above laboratory tests results and the accumulated scores of TCM syndrome（P〈0.05）.Conclusion：Shenbing Fang could provide additional benefits in the treatment of refractory nephrotic syndrome.
