摘 要:目的探讨vWF-Thr789Ala基因多态性与2型糖尿病发生的相关性。方法应用聚合酶链反应—限制性片段长度多态性分析法,测定了57例2型糖尿病患者、24例2型糖尿病患者正常子女和57例正常对照者的Thr789Ala基因多态性;同时应用免疫比浊法测定了血浆vWF活性水平。结果高龄对照组和低龄对照组Thr/Thr、Thr/Ala、Ala/Ala基因型频率分别为0.767、0.167、0.066;0.815、0.111、0.074。2型糖尿病血管并发症组、无血管并发症组及其正常子女组Thr789Ala多态性基因型分布与高龄、低龄对照组比较,差异均无统计学意义(P>0.05)。2型糖尿病血管并发症组的vWF水平高于无血管并发症组(P<0.05);血管并发症组和无血管并发症组vWF水平显著高于高龄对照组(P<0.01);正常子女组vWF水平显著高于低龄对照组(P<0.01)。在2型糖尿病血管并发症组、无血管并发症组和正常子女组中,Thr789Ala基因型间血浆vWF水平比较,差异均无统计学意义(P>0.05);在低龄、高龄对照组中Ala/Ala型vWF水平高于Thr/Thr型和Thr/Ala型,差异均有统计学意义(P<0.05)。结论 vWF-Thr789Ala基因多态性可能与2型糖尿病及其血浆vWF水平无关。Objective To explore the correlation between vWF-Thr789Ala gene polymorphism and type 2 diabetes.Methods Thr789Ala genotype was analyzed by polymerase chain reaction-restriction fragment length polymorphism in 57 patients with type 2 diabetes,24 healthy offsprings of type 2 diabetic patients,and 57 normal control subjects.The level of plasma von Willebrand factor(vWF)was detected by immunoturbidimetry.Results The frequencies of Thr/Thr,Thr/Ala,and Ala/Ala genotypes were 0.767,0.167,and 0.066 in elderly control subjects and 0.815,0.111,and 0.074 in younger control subjects.Compared with control subjects,no significant difference in the genotype distribution was found in type 2 diabetic patients and healthy offsprings(P〈0.05).The vWF level in type 2 diabetic patients with vascular complications was significantly higher than that in those patients without vascular complications(P〈0.05);it was significantly higher in both type 2 diabetic patients with vascular complications and those without vascular complications than in the elderly control subjects(P〈0.01);and it was significantly higher in healthy offsprings than in the younger control subjects(P〈0.01).There was no significant difference in vWF level among Thr789Ala genotypes in type 2 diabetic patients with or without vascular complications and in healthy offsprings(P〈0.05).The vWF level was significantly higher in Ala/Ala genotype than in Thr/Thr and Thr/Ala genotypes in both elderly and younger control subjects(P〈0.05).Conclusion Thr789Ala gene polymorphism may not correlate with the plasma level of vWF in type 2 diabetic patients.
