新肝癌标志物高尔基体蛋白GP73在肝癌血清学诊断中的应用

摘　　要：目的探讨定量测定血清中的高尔基体蛋白(GP73)对肝癌诊断的临床意义。方法应用双抗体夹心酶联免疫定量测定方法检测肝癌、慢性肝炎血清中GP73的含量,应用临床化学发光法测定甲胎蛋白(AFP)为参比。结果在104例肝癌标本中,GP73的阳性率达到81.7%,高于AFP的阳性率62.5%,差异有统计学意义(P<0.05),在226例慢性肝炎血清中,GP73阳性率为31.41%,稍低于AFP阳性率35.8%;GP73诊断良恶性肝病的ROC曲线线下面积达到0.799,对比AFP诊断良恶性肝病的ROC曲线线下面积为0.785;在AFP阴性的39例肝癌中有28个GP73阳性(71.79%);肝病血清中的GP73含量高低与转氨酶ALT、AFP含量都无相关性;在70例其他肿瘤中有2.90%阳性率检测GP73,但在55份健康人标本中未检出。结论 GP73可以作为恶性肝病的独立指标与AFP互补应用于临床。OBJECTIVE The discuss the clinical significance of quantitative determination of the Golgi protein GP73 in the serum on the diagnosis of liver cancer.METHODS The double-antibody sandwich ELISA quantitative measurement methods were used for detection of serum GP73 level in liver cancer and chronic hepatitis,and clinical chemiluminescence of alfa-fetoprotein（AFP） used as a reference.RESULTS In 104 hepatocellular carcinoma specimens,the positive rate of GP73 reached 81.7%,higher than the AFP positive rate（62.5%）,with a significance difference（P0.05）.In 226 cases of chronic hepatitis specimens,GP73 positive rate was 31.41%,slightly lower than the AFP positive rate（35.8%）.The liver ROC area under curve for GP73 diagnosis of benign and malignant disease was 0.799 vs 0.785 for AFP diagnosis.In AFP negative 39 cases of hepatocellular carcinoma the GP73 positive was 28（71.79%）.The serum GP73 level in liver diseases was untelated and the ALT,AFP levels.GP73 detected in 70 other tumor cases（2.90%）,and none in the and 55 healthy persons.CONCLUSIONS GP73 can serve as an independent index for malignant liver disease and be concomitant with AFP for clinical use.
