摘　　要：目的探讨结核分枝杆菌Rv3872抗原在结核病血清学诊断中的价值。方法纯化Rv3872蛋白并作为抗原检测120例确诊的肺内结核病患者、119例确诊的肺外结核患者和127例卡介苗接种后的健康小儿体检者血清中的抗体水平。结果 Rv3872为抗原建立的ELISA检测方法可成功区分肺内结核、肺外结核和BCG接种的健康人群,其在肺内和肺外结核检测中的灵敏度分别为97.5%和95.0%,检测特异性为98.4%。结论 Rv3872应用于结核病血清学诊断具有极高的灵敏度和特异性,具有较高的应用价值。OBJECTIVE To explore the application value of the Mycobacterium tuberculosis Rv3872 antigen in the serodiagnosis of tuberculosis （TB）.METHODS The Rv3872 protein was purified and employed as an antigen to detect the anti-Rv3872 serological levels in 120 patients diagnosed with pulmonary TB,119 patients with extra-pulmonary TB and 127 healthy BCG-vaccinated children undergoing routine physical examinations.RESULTS The Rv3872 antigen-based ELISA assay was able to discriminate the patients with pulmonary TB,extra-pulmonary TB and the healthy BCG-vaccinated group.The sensitivity of Rv3872 in the detection of pulmonary TB and the extra-pulmonary TB was 97.5% and 95.0%,respectively,while the specificity turned out to be 98.4%.CONCLUSION Rv3872,which possesses extremely high sensitivity and specificity,has a high application value in the serodiagnosis of TB.
