颈动脉内膜中层增厚患者血清基质金属蛋白酶-9、高敏C-反应蛋白变化的研究

摘　　要：目的:探讨基质金属蛋白酶-9(MMP-9)、高敏C-反应蛋白(hs-CRP)与颈动脉内膜中层厚度(CI-MT)、斑块特征的相关性。方法:选择180例患者,采用高分辨血管外超声法检测CIMT及粥样斑块;根据CIMT及斑块情况,分为内膜正常组、内膜弥漫性增厚组、稳定性斑块组、不稳定性斑块组。用ELISA方法测定各组血清中MMP-9、hs-CRP浓度,分析超声结果与血清MMP-9、hs-CRP浓度的关系。结果:内膜正常组、内膜弥漫性增厚组、稳定性斑块组、不稳定性斑块组各组之间相比,血清中MMP-9、hs-CRP浓度逐渐增高,各组之间差异均有统计学意义(P<0.05)。结论:血清中MMP-9、hs-CRP浓度增高与CIMT的厚度、斑块的不稳定性关系密切。Objective：To investigate the relationship between matrix metalloproteinase-9,high sensitive C-reactive protein and carotid intima-media thickness.Method：180 patients were selected.High-resolution ultrasound was used to scan carotid,brachial arteries of all patients in order to check and measure carotid intima-media thickness （CIMT）,plaques of carotid arteries and diameter of brachial arteries at rest.Patients were divided into four groups：the normal carotid intima group,the difficuse proliferative carotid intima group,the stable plaque group and the unstable plaque group.Serum levels of MMP-9,hs-CRP were determined with immunological method.The relationship between the results measured by ultrasound instrument and the concentrations of MMP-9,and hs-CRP was analyzed.Result：The serum levels of MMP-9 and hs-CRP in the proliferative carotid intima group were higher than in the normal carotid intima group （P〈0.01）.The serum levels of MMP-9 and hs-CRP in the unstable plaque group and in the stable plaque group were higher than those of difficuse proliferative carotid intima group （P〈0.05）.The serum levels MMP-9 and hs-CRP in the unstable plaque group were higher than those of the the stable plaque group （P〈0.05）.Conclusion：The increased concentrations of MMP-9 and hs-CRP were closely correlated with the increment of carotid intima-media thickness and the unsteady state of plaque.
