初产妇产前焦虑、抑郁与分娩方式及住院医疗费用的相关性研究

摘　　要：目的:探讨正常初产妇产前焦虑、抑郁的发生及其与分娩方式、住院医疗费用的相关性。方法:选取81名本院产科待产的无医学剖宫产指征的正常初产妇,进行综合医院焦虑抑郁自评量表(HADS)及人口学资料问卷调查,出院后追踪分娩方式及住院费用。结果:健康初产妇中焦虑、抑郁症状检出率分别为11.11%,23.46%,远高于正常人群;初产妇年龄、家庭月收入、怀孕意愿与产前焦虑、抑郁症状检出比较,差异有统计学意义(P<0.05);产前焦虑抑、郁者剖宫产率为52.63%,正常产妇剖宫产率为27.42%,两者差异有统计学意义(P<0.05);产前有焦虑、抑郁心境者住院费用与正常心境产妇的住院费用比较,差异有统计学意义(P<0.05);但不同心境同一分娩方式的初产妇住院费用之间,差异无统计学意义。结论:初产妇产前心理状态与其分娩方式及住院费用有一定的联系。Objective：To discuss the incidence of anxiety and depression in primiparas and their relationship with delivery mode and hospitalization expenses. Methods：81 normal primiparas with no cesarean section history in our hospital were randomly selected to do the investigation through questionaire to record the HADS and their demographic data. The delivery mode and hospitalization expenses were followed up after the patients were discharged. Results：The detection rate of anxiety and depression was 11.11%,23.46% respectively in normal primiparas that was far higher than normal group of people. Obvious difference was found between the age,month income of family and pregnancy desire and detection rate of prenatal anxiety and depression （P〈0.05）. The rate of cesarean section of prenatal depression was 52.63%,with obvious difference（P〈0.05） with that of normal without cesarean section history,27.42%（P〈0.05）. There was obvious difference of hospitalization expenses between patients with prenatal anxiety and depression mood and normal mood （P〈0.05）. But there was no statistical difference of hospitalization expenses between the different mood of primiparas who adopted the same delivery mode. Conclusion：There is certain relationship between the psychological status,delivery mode and hospitalization expenses.
