摘  要:目的:观察等长运动负荷试验对甲状腺功能亢进性心脏病(HHD)患者的影响.方法:31例HHD患者按相近年龄、性别和体质量指数与健康体检者(健康对照组)进行1:1配对,治疗前进行等长运动负荷试验,并与健康对照组比较.结果:静息条件下,HHD组左室舒张和收缩功能指标均明显差于健康对照组(P〈0.05~0.01).等长运动后,HHD组左室舒张早期最大流速(E峰)/舒张晚期最大流速(A峰)[(0.80±0.12)∶(1.21±0.23)]和左房射血前期[LAPEP,(81.95±11.28) ms∶( 86.74±12.66) ms]明显低于健康对照组(P〈0.05),而左房射血时间[LAET ( 138.43±27.92) ms∶( 121.42±26.5) ms]和左室等容舒张时间[LVRT( 96.47±14.51)ms∶( 85.97±12.02) ms]均明显高于健康对照组(P均〈0.05);HHD组左室收缩末内径[LVESd( 53.84±6.58) mm∶( 45.16±5.29) mm]、左室舒张末期内径[LVEDd( 63.25±7.95) mm∶( 54.12±6.25) mm]显著扩大、心脏指数[CI (2.66±0.57) L/min·m^2∶(2.38±0.49) L/min·m^2]和左室质量指数[LVMI(115.39±10.35) g/m^2∶( 87.04±8.36) g/m2]均明显高于健康对照组,而周径纤维缩短速度[VCF( 0.88±0.09) cir/s∶( 1.02±0.13) cir/s]和左室射血分数[LVEF( 45.72±6.04) %∶(57.80±6.99%)]均明显低于健康对照组(P均〈0.05).结论:等长运动运动负荷试验是评估甲状腺功能亢进性心脏病患者心功能的有效方法.Objective: To observe the effect of isometric exercise stress test in patients with hyperthyroid heart disease (HHD). Methods: A total of 31 HHD patients (HHD group) were matched with healthy examinees (normal control group) by one to one in the near age, sex and body mass index. Isometric exercise stress test was performed in both groups before treatment, and the results in the HHD group were compared with those in normal control group. Resuits: Most indexes of left ventricular systolic function and left ventricular diastolic function in the HHD group were obviously worse than those in the control group under resting conditions (P〈0.05-〈0.01 respectively). After isometric exercise the values of E/A ratio [ (0.80±0.12) vs. (1.21±0.23)] and left atrial preejection period [LAPEP (81.95 ± 11.28) ms vs. ( 86.74 ± 12.66) ms] in HHD group were obviously lower than those in normal control group, the left atrial ejection time [LAET (138.43±27.92) ms vs. (121.42±26.5) ms] and left ventricular relaxation time [LVRT (96.47±14.51) ms vs. (85.97±12.02) ms] in HHD group were obviously higher than those in normal control group P〈0.05 all. After isometric exercise the values of left ventricular end-systolic dimension [LVESd (53.84±6.58) mm vs. (45.16 5.29) mm], left ventricular end-diastolic dimension [LVEDd (63.25±7.95) mm vs. ( 54. 12±6.25) mm], cardiac index [CI ( 2.66±0. 57) L/min · m^2 vs. (2.38±0.49) L/min · m^2] and left ventricular mass index [LVMI (115.39±10.35) g/m^2 vs. (87.04±8.36) g/m^2] in the HHD group were obviously higher than those in normal control group, and the values of velocity of circumferential fiber shortening [VCF ( 0. 88± 0.09) cir/s vs. ( 1.02±0. 13) cir/s] and ejection fraction ] EF (45.72±6.04) % vs. (57.80±6.99%) in HHD group were obviously lower than those in normal control group (P〈0.05 all).
