摘 要:目的了解短时受精对体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)结局的影响。方法选择在本生殖医学中心接受常规IVF治疗的248个周期,通过一定条件限制将其分为短时受精组和过夜受精组,将248个常规IVF周期中的卵子进行配对资料(同一IVF周期同时进行短时受精和过夜受精者,共20个周期)和非配对资料(1个IVF周期只采用1种方式受精者,过夜受精组177个周期,短时受精组51个周期)研究,比较短时受精组和过夜受精组的正常受精(2PN)率、多精受精率、卵裂率、优质胚胎率及临床妊娠率。结果无论配对还是非配对资料,短时受精组的正常受精(2PN)率、多精受精率、卵裂率、优质胚胎率、临床妊娠率等与过夜受精组比较均无统计学差异(P>0.05);在非配对资料中,短时受精组的移植优胚率(77.33±36.69)%明显高于过夜受精组(64.04±41.85)%(P<0.05),而在配对资料中,用于移植的优质胚胎率短时受精组和过夜受精组无统计学差异(P>0.05)。结论短时受精是一种安全、有效的受精方式,但不能改善胚胎质量和妊娠结局。Objective To investigate the effects of short gamete (sperm-oocyte) co-incubation on outcomes of in vitro fertilization and embryo transfer (IVF-ET).Methods All oocytes from 248 IVF-ET cycles in our department from October 2008 to September 2009 were studied in pair and non-pair design.They were divided with some condition into shortened insemination group and overnight insemination group.Their IVF outcomes were analyzed.Results All pair and non-pair data showed that there was no significant difference in the number of retrieved oocytes,percentage of oocytes at MⅡ stage,diploid fertilization rates (two pronuclei,2PN),polyspermy (2PN) rates,cleavage rates,high quality embryo rates and the pregnancy rates between shortened insemination group and overnight fertilization group(P0.05).The high quality embryo rates used for ET in shortened insemination group were significantly higher than those in overnight insemination group in the non-pair data(P0.05),but no significant difference between shortened insemination and overnight fertilization groups was found in the pair data(P0.05).Conclusion Short gamete co-incubation is reliable and efficacious.However,it could not improve embryo quality.
