2型糖尿病合并冠心病胰岛素抵抗与TNF-α、IL-6相关性

摘　　要：目的探讨2型糖尿病合并冠心病的患者胰岛素抵抗程度及IL-6、TNF-α的水平变化在糖尿病冠心病发生、发展中的作用。方法比较60例2型糖尿病并发冠心病患者与60例糖尿病无并发症患者以及60例健康者的生化指标和放免法测定的血清IL-6、TNF-α、Homa-IR指数的差异性。结果 2型糖尿病并发冠心病组与无并发症组IL-6、TNF-a和Homa-IR水平比较均显著升高(P<0.01)。且IL-6与Homa-IR、IL-6与TNF-α、Homa-IR与TNF-α成正相关。结论 IL-6、TNF-α等炎症标志物的高表达促进了糖尿病冠心病的发生及发展,且与胰岛素抵抗程度呈正相关;检测糖尿病患者血清IL-6、TNF-α、Homa-IR的水平有利于监测其病情、评价疗效及预防并发症,具有重要的临床意义。Ahn To investigate the corrrelation of levels , IL - 6 and TNF-α and Homa-IR with insulin resistance in type 2diabetic patients complicated with coronary heart disease （CHD） . Methods Serum levels of IL-6,TNF-α, Homa-IR in 60 type 2 daibetie patietns complicated with CHD , 60 type 2 diabetic patients without CHD and 60 healthy people were masured and compared. Results The levels of IL-6,TNF-α and Homa-IR were significantly higher in patients with CHD than those without CHD （P〈0.01）. Conclusion The high expression ]eves of IL-6,TNF-α/Homa-IR play important role in the development of coronary heart disease in type 2 diabetic patients and monitoring the expression levels of IL-6,TNF--α,Homa- IR might be helpful in assessment of treatment effcaey and prediction of complications.
