摘 要:目的 根据北京市2004~2008年围产儿多指(趾)畸形的发病情况,分析北京地区围产儿多指(趾)畸形的相关危险因素,为预防和控制多指(趾)畸形的发生提供依据.方法 应用SPSS 12.0软件对北京市2004~2008年多指(趾)畸形监测资料进行统计分析.结果 北京市围产儿5年间多指(趾)畸形发生率为16.88/万,年度发生率呈逐年上升趋势;多指(趾)患儿中合并其他畸形占6.33%,排名第1为总唇裂;多指(趾)的相关危险因素与母亲低龄、围产儿性别、出生体重及分娩孕周有关(χ^2别为14.242、10.706、79.292、18.514,均P〈0.05),与母亲户籍地、胎儿出生季节等无关(均P〉0.05).结论 北京市多指(趾)的发病率在我国处于较高水平,其原因需要进一步确定,以便减少孕产妇危险因素的暴露.临床医生应加强对多指(趾)患儿的查体,注意是否合并其他畸形,并加强对遗传性疾病的诊断能力.Objectives To investigate prevalence rate of perinatal polydactyly in Beijing in a period from 2004 to 2008 and to analyze the related risk factors of the disease so as to provide a basis for evidence-based prevention and control of incidence of the disease. Methods The birth defects surveillance data ( perinatal polydactyly) in Beijing city in a period from 2004 to 2008 were analyzed statistically by using SPSS 12.0 software. Results The prevalence rate of perinatal polydactyly in Beijing in the 5-year period was 16.88 per 10 000 and in annual prevalence there was a year-by-year increasing tendency. The infants with polydactyly associated with other malformations accounted for 6.33%, of which, the first associated malformation was cleft lip with/without cleft palate. The incidence of polydactyly was related to lower age of the pregnant women, sex of fetus, birth weight and gestational age at birth ( Chi-square value was 14. 242, 10.706, 79. 292 and 18. 514 respectively, and all P values were 〈 0.05 ), while that was not related to mother' s registered permanent residence and fetal birth seasons ( both P 〉 0.05). Conclusion The local prevalence of perinatal polydactyly in Beijing was at a high level compared with the national average, and the causes need to be further determined in order to reduce maternal exposure to the risk factors. Clinicians should pay more attention to physical examination for those infants with polydactyly to determine whether the infant is associated with other malformations to raise their diagnostic ability for genetic diseases.
