经伤椎椎弓根自体骨植骨结合椎弓根内固定系统治疗胸腰椎爆裂性骨折

摘　　要：目的评价在椎弓根内固定系统的基础上经伤椎椎弓根植入自体骨至椎体内的疗效。方法采用经伤椎椎弓根自体骨植骨,椎弓根螺钉复位内固定治疗胸腰椎爆裂性骨折20例,术前、术后摄X线片及CT检查,观察椎体高度有无丢失,内固定有无松动、断裂。结果骨折愈合良好,未出现椎体高度再丢失,无内固定松动、断裂。结论经伤椎椎弓根自体骨植骨联合椎弓根内固定系统治疗胸腰椎爆裂性骨折,杜绝异体骨植骨的排异反应,可恢复伤椎椎体高度,重建脊柱前中柱的稳定性,防止后期椎体高度的再丢失和内固定的松动、断裂,值得临床推广。Objective To evaluate the pedicle fixation system based on the injury of vertebral vertebral arch grounding into autogeneous bone to vertebral body curative effect and advantages.Methods The injury vertebral pedicle autogenous grafts,pedicle screws reduction treatment of thoracolumbar burst fracture,preoperative and postoperative 20 cases of perturbation X-ray and CT were observed whether vertebral height loss,or loose and fracture fixation.Results Fractures healed well,not to lose height,vertebral fracture fixation loose.Conclusion The injury vertebral pedicle autogenous grafts joint pedicle fixation system treatment of thoracolumbar burst fracture,eliminate allograft bone graft rejection,may restore injury vertebral vertebral height,rebuild spinal column,the stability in the former stage of vertebral body height to prevent loss of fixation and again loose,fracture.
