摘  要:目的探讨孕24+0~28+0周妊娠期糖尿病(GDM)者胎儿与非GDM者胎儿相比是否出现加速生长,且反映胎儿加速生长的超声生物测量值是否与胎儿出生体重有关。方法 2007年8月至2008年12月于四川大学华西第二医院,对孕24+0~28+0周的孕妇103例(GDM组38例、非GDM组65例)胎儿的双顶径(BPD)、股骨长(FL)、头围(HC)、腹围(AC)、肝脏长度(LL)、腹壁皮下组织厚度(SCTT)进行超声生物测量。孕妇行50g葡萄糖激发试验(GCT)及75g口服葡萄糖耐量试验(OGTT)以筛查和诊断GDM。随访纳入的孕妇至分娩。结果 GDM组与非GDM组LL分别为(35.49±4.25)mm及(32.80±3.18)mm,两者差异有统计学意义(t=-2.239,P=0.027)。多元回归分析显示,LL与胎儿出生体重的密切相关(t=2.634,P=0.01)。结论孕24+0~28+0周期间GDM者胎儿与非GDM者胎儿相比其LL出现加速生长;胎儿LL与其出生体重密切相关。Objective To observe whether fetal growth acceleration happens between 24 +0 - 28+0 weeks' gestation in pregnancy affected by GDM and whether the growth acceleration has an effect on birth weight. Methods Women of gestational age between 24 +0 - 28+0 weeks were included. Fetal ultrasound biometry measurements of biparietal diameter (BPD), femur length (FL), head circumference (HC), abdominal circumference (AC), liver length (LL), and sub- cutaneous soft tissue thickness (SCTT) were done. GDM was confirmed by 50g GCT and 75g OGTT. Women were followed up until pregnancy outcome data were collected. Results 103 pregnant women were included, 38 of which were grouped as GDM while the other 65 were grouped as Non - GDM. Mean LL of GDM group and Non - GDM group was (35.49 ± 4. 25 ) mm and (32. 80 ± 3. 18) mm, respectively. There was significant difference in LL (t = - 2. 239, P = 0. 027 ) between two groups. Only LL was selected into the regression model of the multiple regression analysis between birth weight and fetal ultrasound biometry measurements. LL had a significant effect on birth weight ( t = 2. 634, P = 0. 01 ). Conclusions Fetal LL shows growth acceleration between 24 +0 - 28+0 weeks' gestation m pregnancy affected by GDM. Fetal LL has a significant effect on birth weight.
