摘  要:目的:应用舒张期左室内血流传播速度(Vp)和血清脑钠肽(BNP)水平评价原发性高血压患者(EH)左室舒张功能。方法:用彩色M型超声心动图测量19例EH患者和20正常对照者的Vp,采用酶联免疫吸附试验(ELISA)检测血清BNP浓度,并用频谱多普勒法测量二尖瓣口舒张期血流速度E峰、A峰及E/A值,分析其变化并比较几种方法的相关性。结果:高血压组E峰、E/A和Vp明显降低(P<0.05),A峰和BNP明显升高(P<0.05)。高血压组BNP浓度与Vp呈负相关(r=-0.748,P<0.05),BNP浓度与E/A无明显相关性(r=-0.185,P>0.05),Vp与E/A无明显相关性(r=-0.048,P>0.05)。结论:BNP和Vp均能反映高血压患者舒张功能的变化,两者结合可为无创性估测无症状性高血压病左室舒张功能障碍提供一个好的途径。Objective:To evaluate the left ventricular diastolic function in patients with essential hypertension by the propagation velocity(Vp) of early flow into the left ventricular and the levels of brain natriuretic peptide(BNP).Methods:19 patients with essential hypertension and 20 healthy control were enrolled into the study.Vp was measured by color M-mode Doppler echocardiography.The levels of BNP were measured by ELISA.The peak flow velocity velocity E,A of mitral inflow by Doppller flow imaging were detected.The correlation between these Doppler parameters and BNP were analysed.Results:E,E/A and Vp in patients with hypertension were lower than those in control group(P 0.05),whereas A and BNP were increased(P 0.05).The serum concentration of BNP were negatively correlated with Vp in the patients with hypertension(r =-0.748,P 0.05).E/A were not correlated with Vp and BNP(r =-0.185 and-0.048,all P 0.05).Conclusion:Vp and BNP can reflect the changes of diastolic function in patients with hypertension.It may provide a good way to non-invasively evaluate the asymptomatic left ventricular diastolic dysfunction in patients with hypertension.
