摘  要:目的探讨不同换药间隔时间对清创缝合术后患者伤口愈合的影响。方法将226例清创缝合术后需要换药的患者随机分为观察组116例和对照组110例,两组患者清创缝合术后均按常规抗感染治疗,观察组在清创缝合术后第1d、第2d进行首次、第2次换药,然后间隔3d换药1次,直至伤口拆线,对照组清创缝合术后按常规间隔ld换药1次,直至伤口拆线。比较两组患者清创缝合术后换药时伤口痂皮损伤发生率及伤口良好愈合率。结果观察组患者换药揭除敷料时肉芽组织或伤口痂皮损伤发生率为1.7%,对照组为10.9%,两组比较,χ2=8.20,P〈0.01,差异具有统计学意义;观察组患者伤口良好愈合率为91.4%,对照组为77.3%,两组比较,χ2=8.58,P〈0.01,差异具有统计学意义。结论换药间隔时间与清创缝合术后伤口的愈合有关,改良换药时间有助于患者创面的愈合,同时减少患者的痛苦和经济负担,值得临床推广应用。Objective To explore the effect of dressings changing at variant time intervals on the intentions in patients undergoing debridement and suturing. Methods 226 patients undergoing debridement and suturing were randomly divided into the study group(n = 116) and the control one(n = 110) . All the patients were treated by anti-infection routinely. In the study group,the first and second changes of dressings were done on day one and two,respectively and then the dressings changing once every three days until the wounds were healed. The crust damage rate at changing dressings and the healing rate were compared between the two groups. Results The crust damage rates at changing dressings in the study group and the control group were 1.7% and 10.9%,respectively. The difference was significant(χ2 = 8.20,P 0.01) . The healing rates in the study group and the control one were 91.4% and 77.3%,respectively. The difference was significant(χ2 = 8.58,P 0.01) . Conclusion The time for changing dressings is related to the healing of wounds. So reasonable arrangement of time for changing dressings is helpful for healing of wounds,relief of their pains and decrease of their economical burden.
