摘 要:目的观察6%羟乙基淀粉(万汶130/0.4)行急性等容血液稀释对脑缺血再灌注兔血清肿瘤坏死因子-α(TNF-α)、白介素-1(IL-1)、白介素-6(IL-6)和S100-β水平的影响。方法新西兰大白兔24只随机分为3组,其中假手术组(S组,n=8);缺血-再灌注组(IR组,n=8);血液稀释-缺血-再灌注组(HIR组,n=8)。假手术组行假手术,缺血再灌注组和血液稀释缺血再灌注组经右大脑中动脉栓塞造成2小时局灶性脑缺血后再灌注;脑缺血再灌注模型参照ZeaLonga的线栓法。血液稀释缺血再灌注组从右股动脉放血,同时从股静脉补入等量6%羟乙基淀粉,20min内完成,稀释目标Hct值为30%。于缺血前30min(To)、再灌注即刻(T1)、再灌注3h(T2)、再灌注6h(T3)、再灌注24h(T4)用酶联免疫吸附法(ELISA法)检测颈静脉血清IL-1、IL-6、TNF-α、S100-β含量。结果①在T1~T4时间点,IR组和HIR组的血清TNF-α、IL-1、IL-6浓度明显高于S组(P<0.05),并且IR组比HIR组更高②与T0时比较,三组T1~T4时TNF-α、IL-1、IL-6浓度显著高于T0时(P<0.05)。③在T1~T4时间点,IR组和HIR组的血清S100-β浓度明显高于S组(P<0.01),且IR组比HIR组更高(P<0.05)。结论应用6%羟乙基淀粉血液稀释可降低脑缺血再灌注血清TNF-a、IL-1、IL-6、S100-β的表达。【Objective】To investigate the effect of acute normovolemic hemodilution with 6% hydroxyethyl starch(HES 130 /0.4) on the serum levels of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1),inter-leukin-6(IL-6) and S100-β protein after cerebral ischemia reperfusion in rabbits.【Methods】24 New Zealand white rabbits were randomly divided into 3 groups:sham operation group(S group,n = 8),ischemia repefusion group(IR group,n = 8),hemodilution ischemia repefusion group(HIR group,n = 8).Sham operation group was undergoning sham operation;ischemia reperfusion group and hemodilution ischemia reperfusion group were undergoing thread embolism of the right middle cerebral artery occlusion(MCAO) to cause focal ischemia for 2 hours and then were undergoing reperfusion;Rabbits'cerebral ischemia reperfusion injury model was established by ZeaLonga's thread method.Before the ischemic period,the HIR group was undergoing acute normovolemic hemodilution(ANH) to achieve a hematocrit of 30% by stepwise blood withdraw from right femoral artery and normovolemic hemodilution with 6% HES from the femoral vein in 20 min.The jugular vein blood samples were taken for the enzyme-linked immunosorbent assay(ELISA) measurements of IL-1,IL-6,TNF-α and S100-βat before ischemia 30min(T0),immediate reperfusion(T1),reperfusion 3h(T2),reperfusion 6h(T3) and reperfusion 24h(T4).【Results】① The serum TNF-α,IL-1,IL-6 levels in the T1 ~ T4 time points were significantly higher in IR group and HIR group than those in S group(P〈0.01).and the TNF-α,IL-1 and IL-6 serum levels were significantly higher in IR group than those in HIR group in the T1 ~ T4 time points(P 0.05) ;②Compared to T0,the the concentration of serum TNF-α,IL-1 and IL-6 were increase at T1 ~ T4 in IR group and the HIR group.(P 0.05) ;③The serum S100-β levels in the T1 ~ T4 time points were significantly higher in IR group and HIR group than those in S group(P〈0.01),and the S1
