摘  要:目的探讨老年男性骨质疏松症者血中细胞因子,如白细胞介素-6(IL-6)、转化生长因子-β_1(TGF-β_1)和性激素,如雌二醇(E_2)、睾酮(TT)水平的变化及其临床意义。方法采用超声骨密度仪、放射免疫法和化学发光法测定65例老年男性(年龄大于或等于60岁)和30例健康中青年男性(年龄小于60岁)超声振幅衰减(BUA),以及血清中IL-6、TGF-β_1、E_2、TT等指标的水平。结果随着年龄的增长,老年男性BUA、TGF-β_1、E_2、TT水平呈下降趋势,IL-6水平呈上升趋势,与健康对照组比较,差异有统计学意义(P<0.001~0.05)。老年男性骨质疏松症组及骨量减少组BUA及TGF-β_1、E_2血清水平明显低于老年男性正常骨量组(P<0.001~0.05)。老年男性组中,TGF-β_1和E_2与BUA呈正相关(r分别为0.28和0.47,P<0.05),IL-6与E_2和BUA呈负相关(r分别为-0.27和-0.36,P<0.05)。结论老年男性骨密度降低与性激素水平降低及细胞因子水平变化等多种因素密切相关。Objective To investigate the change of cytokine(IL-6.TGF-β1) and sex hormone(estradiol-E2, testosterone-TT) in the blood from the aged male patients with osteoporosis and it's clinical significance. Methods The ultrasound bone densitometer, radioimmunoassay,and chemiluminescence assay were used to examine the broadband ultrasound attenuation(BUA), and the serum levels IL-6.TGF-β1.E-2.TT among 65 aged male(≥60 years old)and 30 cases of healthy control(〈60 years old). Results With the increasing of age,the BUA,TGF-β1,E-2 and TT decreased and the IL6 increased in the aged male group with statistical significance (P〈0. 001-0.05) compared with control group. The BUA and serum levels of TGF-β1 and E-2 in aged male with os- teoporosis and reduced bone mass were obviously lower than those of aged male with a normal bone mass(P〈0. 001-0.05). The TGF-131 and E 2 were positively correlated to BUA(r=0. 28,r= 0.47 and P〈0.05) ,whereas the IL- 6 and E-2 were negatively correlated to BUA(r= -0.27,r =-0.36 and P〈0.05). Conclusion The decrease of bone mineral density in aged male was closely correlated to the decrease of the sex hormone and the changes of eytokine levels and so on.
