摘　　要：目的：测定多囊卵巢综合征（Polycystic Ovarian Syndrome,PCOS）患者可溶性细胞间黏附分子-1（Soluble In-tercellular Adhesion Molecule-1,sICAM-1）的水平,探讨sICAM-1与PCOS患者的关系和临床诊治作用。方法：随机选取2005年6月~2007年7月在广西医科大学一附院妇科门诊就诊的PCOS患者54例为PCOS组,以月经正常的非PCOS患者61例为对照组,又以体重指数（BMI）≥24 kg/m2或腰臀比（WHR）≥0.8为肥胖标准将研究对象分为肥胖组和非肥胖组。所有研究对象均于月经第3~5天（闭经者日期不限）清晨空腹抽取静脉血测定血清中sICAM-1和性激素水平,并且测量其身高（m）、体重（kg）、腰围（cm）、臀围（cm）,计算体重指数和腰臀比。结果：①PCOS组血清sICAM-1水平高于对照组（P〈0.05）;②PCOS肥胖组血清sICAM-1水平高于PCOS非肥胖组（P〈0.05）;③多因素分析显示血清sICAM-1水平与年龄、BMI、WHR、睾酮呈正相关。结论：血清sICAM-1与PCOS有密切关联,sICAM-1异常表达在PCOS患者导致心血管疾病发病机理中起一定作用,PCOS患者具有发生心血管疾病的危险性,并且随着年龄、BMI、WHR和睾酮水平增高,PCOS患者患心血管疾病风险也增加。Objective：To detect the level of soluble intercellular adhesion molecule-1（sICAM-1） in patients with polycystic ovary syndrome（PCOS）,explore the relationship between sICAM-1 and PCOS and the value of sICAM-1 in diagnosis and treatment of PCOS.Methods：54 cases with PCOS in the hospital from June 2005 to July 2007 were selected randomly as PCOS group,and 61 cases without PCOS and with normal menstruation were selected as control group,the research objects were divided into obesity group and non-obesity group according to body mass index（BMI）≥24 kg/m2 or waist hip ratio（WHR）≥0.8;the morning venous blood samples of all the research objects were taken to detect the serum levels of sICAM-1 and sex hormones from the third day to the fifth day during menstruation;body height（m）,weight（kg）,waist circumference（cm） and hip circumference（cm） were measured,BMI and WHR were calculated.Results：The serum level of sICAM-1 in PCOS group was higher than that in control group,there was significant difference（P〈0.05）;the serum level of sICAM-1 in PCOS obesity subgroup was higher than that in PCOS non-obesity subgroup,there was significant difference（P〈0.05）;multivariate analysis indicated that there was positive correlation between serum sICAM-1 level and age,BMI,WHR and testosterone.Conclusion：Serum sICAM-1 is related to PCOS closely,abnormal expression of sICAM-1 plays an important role in mechanism of PCOS induced cardiovascular diseases,the patients with PCOS have the risk of cardiovascular diseases,and the risk increases with age,BMI,WHR and testosterone level.
