获卵数对长方案IVF-ET妊娠结局的影响

摘　　要：目的：探讨获卵数对长方案体外受精-胚胎移植（IVF-ET）妊娠结局的影响。方法：回顾性分析2001年7月～2007年12月于中山大学附属第二医院生殖中心行长方案IVF-ET治疗的1 161对不孕夫妇的1 389个获卵周期。按获卵数分为5组：A组（≤5个）,B组（6～10个）,C组（11～15个）,D组（16～20个）,E组（≥21个）。Logistic回归分析获卵数对临床妊娠及累积临床妊娠结局的影响。结果：获卵数与年龄成负相关（r=-0.267,P〈0.001）;Logistic回归排除年龄因素的影响后,A、E组的临床妊娠率较C组明显下降,OR=0.33,0.51,P〈0.05;累积临床妊娠率由A组至D组逐渐升高,D组最高为46.3%,E组略有下降,A、B组相对于C组的OR值分别为0.25和0.70,P〈0.05。D、E组与C组相比差异无显著性。结论：长方案IVF中获取中等数量的卵子能获得最佳的临床妊娠率和累积临床妊娠率。Objective：To explore the effect of number of retrieved oocytes on pregnancy outcomes of cases receiving long protocol in vitro fertilization and embryo transfer（IVF-ET）.Methods：1 389 retrieval cycles of 1 161 couples with infertility who received long protocol IVF-ET in the hospital from July 2001 to December 2007 were analyzed retrospectively,then the cases were divided into 5 groups according to number of retrieved oocytes： A group（≤5）,B group（6～10）,C group（11～15）,D group（16～20） and E group（≥21）;the effects of number of retrieved oocytes on outcomes of clinic pregnancy and accumulated clinic pregnancy were analyzed by logistic regression analysis.Results：There was a negative correlation between number of retrieved oocytes and age（γ=-0.267,P〈0.001）;multivariable logistic regression analysis showed that the clinic pregnancy rates of A group and E group were significantly lower than that in C group（OR=0.33,0.51,P〈0.05） after excluding age factor;the accumulated clinic pregnancy rate increased gradually from A group to D group,the accumulated clinic pregnancy rate of D group was the highest（46.3%）,then decreased slightly in E group,the OR values of A group,B group to C group were 0.25 and 0.70,respectively（P〈0.05）;there was no significant difference between D group,E group and C group.Conclusion：Moderate number of retrieved oocytes may get optimal clinic pregnancy rate and accumulated clinic pregnancy rate in long protocol IVF-ET.
