用PCR技术对妊娠期HPV16和18型母婴垂直传播的研究

摘　　要：目的：研究人乳头瘤病毒（ＨＰＶ）在妊娠晚期和分娩期母婴间的垂直传播。方法：应用聚合酶链反应（ＰＣＲ）方法，随机对５０例妊娠晚期妇女宫颈阴道脱落细胞、外周血单个核细胞（ＰＢＭＣ）、新生儿脐血、羊水、生后１２～４８小时的新生儿咽部分泌物及母亲的乳汁进行ＨＰＶ１６、１８型检测。结果：检出ＨＰＶ１６ＤＮＡ１８例，ＨＰＶ１８ＤＮＡ１７例。新生儿脐血分别是孕妇ＰＢＭＣ中检出ＨＰＶ１６、１８ＤＮＡ者。新生儿咽部分泌物阳性的ＨＰＶ１６、１８ＤＮＡ也分别是孕妇ＰＢＭＣ中检出者。在ＨＰＶ阳性的宫颈分泌物中，其中８例ＨＰＶ１６ＤＮＡ和９例ＨＰＶ１８ＤＮＡ的其它标本中均未检出ＨＰＶ１６、１８ＤＮＡ。结论：妊娠期ＨＰＶ可以在母婴之间垂直传播，而且新生儿感染ＨＰＶ主要来自产前的胎盘血流，也与新生儿吞咽母血有关。说明孕妇ＰＢＭＣ中的ＨＰＶ状态与这种传播有潜在的联系，剖宫产不能完全阻止母婴垂直传播。Objective:To study the possibility of matenal fetal vertical transmission of human papillomavirus (HPV)during pregnancy and delivery.Methods:Specimens of cervicovaginal cells and peripheral blood mononuclear cells(PBMC) were obtained from 50 consecutive pregnant women in the third trimester before rupture of membrane.Cord blood specimens were obtained from the neonates born to these mothers,and specimens of pharyngeal secretions of their neonates were obtained 12～48 hours after birth and maternal breast milk were obtained as well.Amniotic fluid were collected in these pregnant women during cesarean section.The presence of HPV types 16 and 18 deoxyribonucleic acid(DNA) were detected by polymerase chain reaction (PCR).Results:Eighteen cord blood specimens from neonates born to mothers who were positive for PBMC HPV type 16 DNA were found to contain HPV type 16 DNA .So were the seventeen with HPV type 18 DNA.All pharyngeal specimens from neonates born to mothers who were positive for PBMC HPV types 16,18 DNA were found to contain HPV 16,18 DNA.Among HPV positive results of cervicovaginal specimens,there were eight cases HPV 16 DNA and nine cases HPV 18 DNA that were not found HPV types 16,18 DNA in rest specimens.Conclusions:The possible vertical transmition of HPV during pregnancy which is mainly through placental and in some extent associated with gulfing down maternal blood which is infected with HPV.And it indicates the potential association of such transmission with the status of HPV in PBMC.Cesarean section can not protect the neonates against vertical transmission completely.
