摘  要:目的 探讨小儿肠缺血时血清肝、肠脂肪酸结合蛋白的变化.方法 取机械性肠梗阻患儿46例,分为实验一组(手术切除肠管组)11例,实验二组(未手术切除肠管组)23例,实验三组(未手术组)12例,对照组10例,为正常儿童.正常对照组采取外周静脉血1次,肠梗阻患儿分别于入院时、术后(有效治疗)第1天、术后(有效治疗)第3天采取外周静脉血3次.采用酶联免疫吸附测试法检测肠脂肪酸结合蛋白(IFABP)和肝脂肪酸结合蛋白(LFABP).结果 所有实验组患儿入院时IFABP、LFABP较对照组显著增高,以肠坏死组升高最明显;术后第1天、第3天所有实验组IFABP、LFABP全部达到对照组水平;同一时期,相同实验组LFABP水平均较IFABP高.结论 IFABP、LFABP是检测肠缺血、肠坏死较为灵敏的指标,以LFABP更为敏感.Objective To explore the change of plasma intestinal fatty acid -binding protein (IFABP) and liver fatty acid-binding protein (LFABP)in intestinal ischemia children patients. Methods 46 children with mechanical ileus were divided into three experimental groups (EG) according to the different therapeutic selections: EG1 underwent enterectomy with anastomosis because of intestinal necrosis (n = 11), EG2 subjected only celiotomy (n = 23), and patients in the EG3 did not subjected to any operation (n = 12). 10 other healthy children were served as the control group (CG). Venous blood samples were collected from CG for one time, from EGs for three time, namely, after admission, 1st and 3rd day after operation (or effectively therapied). The concentration of serum IFABP and LFABP were measured by enzyme linked immunosorbent assay. Results Compared with the CG, the IFABP and LFABP levels in all the EGs were increased obviously on admission, and the highest was in the EG1. Subsequently, the IFABP and LFABP levels decreased to normal on 1st day after operation (or effectively therapied) in the EGs, while the IFABP and LFABP levels were higher in the EG1 than those in the other two EGs. On the 3rd day, the levels of the IFABP and LFABP recovered to normal in all the EGs. In the same phase, the level of LFABP is higher than the level of IFABP in the same EG. Conclusions Serum IFABP and LFABP are two sensitive markers to detect intestinal ischemia or necrosis, and serum LFABP is more sensitive than IFABP.
