单点硬膜外麻醉辅以舒芬太尼用于MPCNL手术的临床观察

摘　　要：目的观察单点硬膜外麻醉辅以舒芬太尼用于MPCNL(经皮肾微造瘘输尿管镜下钬激光碎石)手术的临床效果。方法将40例行MPCNL手术的患者随机分为单点硬膜外麻醉组(S组)和双点硬膜外麻醉组(D组),每组各20例,监测并记录麻醉前、麻醉后及术毕时的收缩压(SBP)、舒张压(DBP)、心率(HR)及氧饱和度(SPO2)的变化。结果两组患者麻醉后SBP、DBP及HR与麻醉前比较均下降(P<0.05),且麻醉后D组的SBP、DBP及HR下降幅度明显大于S组(P<0.05)。两组的SPO2与麻醉前比较无明显差异(P>0.05)。结论单点硬膜外麻醉辅以舒芬太尼用于MPCNL手术麻醉效果好,对呼吸、循环影响更小,麻醉更平稳。Objective To explore the effect of single point epidural anesthesia combined with sufentanil on MPCNL.Methods 40 patients for MPCNL were randomly divided into single point epidural anesthesia group（group S,n=20）and double point epidural anesthesia group（group D,n=20）.Before the anesthesia and after the anesthesia,the SBP,DBP,HR and SPO2 were recorded.Results The SBP,DBP,HR of two groups after the anesthesia decreased earlier（P〈0.05）.After the anesthesia,the SBP,DBP and HR of D group decreased more significantly than that of the S group（P〈0.05）.There was no significant difference on SPO2 before and after the anesthesia（P〉0.05）.Conclusion Single point epidural anesthesia combined with sufentanil has efficient and intensive analgesic effect and little side effects on respiration and circulation for MPCNL.
