冠状动脉搭桥术前后QT离散度的测定

摘　　要：目的阐明冠状动脉搭桥术（ＣＡＢＧ）对ＱＴ离散度（ＱＴｄ）的影响，并探讨其临床意义。方法回顾性分析了近几年行ＣＡＢＧ术的４２例冠心病患者术前及术后不同时间的心电图，测定其ＱＴｄ和心率校正的ＱＴ离散度（ＱＴｃｄ），同时观察心绞痛缓解程度、心功能改善状况及左室射血分数的变化。结果术前病人的ＱＴｄ，ＱＴｃｄ（８５．６３±２５．４７，９０．６４±２５．７８ｍｓ）均明显高于正常（４０～５０ｍｓ），而术后１ｄ即明显减少（６４．８０±２１．４６，７４．１０±２２．４０ｍｓ，Ｐ＜０．００１），术后１０ｄ基本恢复正常（４０．６７±１４．４３，４３．６７±２１．３２ｍｓ），且这一结果与其它临床指标明显相关。结论ＣＡＢＧ能有效地增加心肌血供、缓解心绞痛，改善心功能；ＱＴｄ，ＱＴｃｄ无论在ＣＡＢＧ术前病情判断还是在术后疗效观察中，都是一项可靠而敏感的指标。Objective To observe the effect of coronary artery bypass graft(CABG) and explore the clinical significance.Method Analysed the electocardiogram of 42 patients with CHD before and after operation and contracted its' QTd and QTcd.At the same time,the authers also observed the remittent degree of angina pectors,the improvement of heart function and the variation of the left ventricular eject fraction.Result Before operation ventercular eject,the QTd and QTcd(85.63±25.47,79.64±25.78ms)of the patients was distinctly higher than normal(40～50ms).The first day after operation,they decreased obviously(64.80±21.46,74.10 ± 22.40ms)and almost normal after ten days(40.67±14.43,43.67±21.32ms ).This result was uniformed with the other standards.Conclusion CABG can distinctly improve the blood suppy of myocardial and the heart function,remit angina pectoris;QTd and QTcd are reliable and sensitive standards both in judging the patients' condition before operation and observing its effect after operation.
