摘  要:目的探讨新生儿呼吸窘迫综合征的影像学特征。方法搜集250例新生儿呼吸窘迫综合征的床旁片及DR片,回顾分析其X线表现。结果①Ⅰ级30例,Ⅱ级87例,Ⅲ级73例,Ⅳ级60例。②X线表现有:肺透亮度降低,肺野内细颗粒状、网格状影,肺野磨玻璃密度,支气管充气征,肺野致密呈"白肺"。③60例Ⅳ级病例中,39例显示支气管充气征,显示率65.00%(39/60);其中DR片35例显示32例,显示率91.43%(32/35),床旁片25例显示7例,显示率28.00%(7/25);支气管充气征的显示率DR片明显高于床旁片,差异有显著性(χ2=25.79,P<0.01)。结论肺野内细颗粒状、网格状影及支气管充气征是新生儿呼吸窘迫综合征主要影像表现,支气管充气征是诊断呼吸窘迫综合征最可靠的征象。Objective To discuss the imaging features of neonatal respiratory distress syndrome. Methods The X ray features of 250 cases of neonatal respiratory distress syndrome were retrospectively analyzed. Results Thirty cases showed Grade Ⅰ, 87 cases Grade Ⅱ, 73 cases Grade Ⅲ and 60 cases Grade Ⅳ. The X ray features showed decreased lung field transmittance, air bronchus and compact lung field looking like "white lung". In 60 cases of Grade Ⅳ, 39 cases showed air bronchus, accounting for 65.00%(39/60), DR film accounting for 91.43% (32/35) and bedside film accounting for 28.00%(7/25). The display rate of DR film was higher than that of bedside film in air bronchus, with a significant difference ( χ2=25.79, P〈0.01 ). Conclusion The typical imaging findings in neonatal respiratory distress syndrome are decreased lung field transmittance and air bronchus, and air bronchus is the most reliable sign in the diagnosis of neonatal respiratory distress syndrome.
