摘  要:目的探讨2型糖尿病伴心脑血管病变患者的脂代谢紊乱、高血压与临床特点的关系。方法将504例住院的2型糖尿病患者分为两组,糖尿病并心脑血管病变组156例,糖尿病无心脑血管病变组348例,比较两组的临床观察指标(血糖、HbA1C、C肽、血脂、血压),筛选心脑血管病变的独立危险因素。结果糖尿病并心脑血管病变组年龄、HbA1C、C肽、TC、LDL-C、收缩压,均显著高于无心脑血管病变组(P<0.05或P<0.01)。多因素非条件Logistic回归分析结果显示,年龄、TC、收缩压、餐后2h C肽是2型糖尿病并心脑血管病变的独立危险因素。结论防治糖尿病,采取控制血糖、血脂、血压,改善胰岛素抵抗等综合措施,可延缓和减轻糖尿病心脑血管病变的发生发展。Objective To explore the relationship between lipid metabolism and hypertension and clinical features in type 2 diabetes with cardiovascular and cerebrovascular diseases. Methods We divided 504 hospitalized patients with type 2 diabetes into two groups: group of diabetes with cardiovascular and cerebrovascular diseases (156) and group of diabetes without cardiovascular and cerebrovaseular diseases (348). The indicators including blood glucose, HbA1C, C peptide, lipids and blood pressure were compared, and the independent risk factors were screened for cardiovascular and eerebrovascular diseases. Results In the group of diabetes with cardiovascular and cerebrovascular diseases, age, HbA1C, C peptide, TC, LDL-C and systolic blood pressure were significantly higher than those in the group of diabetes without cardiovascular and cerebrovascnlar diseases(P〈0.05 or P〈0.01 ). Non-conditional logistic regression analysis showed age, TC, systolic blood pressure and postprandial 2hC peptide of type 2 diabetes with cardiovascular and cerebrovascular diseases were independent risk factors. Conclusion Controlling blood glucose, blood lipids, blood pressure and insulin resistance and taking other comprehensive measures to control diabetes can delay and reduce the occurrence of cardiovascular and cerebrovascular diseases.
