摘 要:目的:探讨针刺对青光眼视神经的保护作用及作用机制。方法:以大耳白兔为实验动物,通过前房内注射复方卡波姆溶液造成实验性青光眼模型。28d后,通过滤过性手术使眼压恢复正常。20只(40只眼)大耳白兔分层随机分为模型组、针刺组、神经营养剂组,并设正常对照组,每组5只,眼数为10只。针刺组穴取双侧"球后""风池""行间",每日早、晚各针1次;神经营养剂组给予维生素B1100mg、维生素B12500μg肌肉注射,每日1次;其他组不做治疗处理。治疗4周后,观察各组兔视网膜组织凋亡抑制基因(Bcl-xl)、脑源性神经营养因子(BDNF)表达的变化。结果:实验结束时,与模型组相比,针刺组视网膜组织Bcl-xl、BDNF表达分别为(31.20±5.97)个/mm2、(6.3±1.89)个/mm2,显著高于模型组的(26.70±4.32)个/mm2、(4.0±1.89)个/mm2,两组相比,差异有统计学意义(均P<0.05)。结论:针刺能上调青光眼模型兔视网膜Bcl-xl和BDNF的表达,从而保护高眼压损害的视神经。Objective To explore the protection effects of acupuncture on glaucomatous optic nerve damage and its mechanism. Methods Experimental glaucoma model was induced by intracameral injection of compound Carbomer solution in rabbits. After 28 days, ocular tension returned to normal by filtration surgery. Twenty rabbits (40 eyes) were randomly divided into a model group, an acupuncture group,a neurotrophy group and a nomal control group, 5 cases in each group. The acupuncture group was treated with acupuncture at bilateral "Qiuhou" (EX-HN 7) ,"Fengehi" (GB 20) and "Xingjian" (LR 2), twice a day. The neurotrophy group was treated with intramuscular injection of Vitamin B1 (100 mg) and Vitamin Bl2 (500 μg), once a day, and the other groups with no treatment. The expressions of Bcl-xl and BDNF in rabbits retina were observed after 4 weeks. Results At the end of the experiment, the positive expression cells of Bcl-xl and BDNF were (31.20±5.97) per mm^2 and (6.3±1.89) per mm^2 in the acupuncture group, being significantly higer than (26.70±4.32) per mm^2 and (4.0±1.89) per mm^2 in the model group(both P〈0.05). Conclusion Acupuncture can raise the expression of Bcl-xl and BDNF of retina, so as to prevent optic nerve damage caused by intraocular hypertension.
