摘 要:目的观察奥美沙坦对高血压患者颈动脉内膜中层厚度及血管内皮功能的影响。方法 60例高血压病患者随机分为对照组(n=30)及奥美沙坦组(n=30),观察用药前后的颈动脉内膜-中层厚度(IMT)、肱动脉内皮依赖性舒张功能(FMD)、一氧化氮(NO)及内皮素-1(ET-1)的变化。结果治疗前后,奥美沙坦组及对照组血压无显著性差异;奥美沙坦治疗后,IMT、ET-1显著降低(P<0.05),FMD及NO水平显著增加(P<0.05);与对照组比较,IMT及ET-1显著降低(P<0.05),FMD及NO显著增加(P<0.05)。结论奥美沙坦可明显改善高血压患者内皮功能,具有独立于降压作用以外的抗动脉粥样硬化作用。Objective To study the effect on intimamedia thickness (IMT) , and endothelial function on patients with hypertension. Methods Sixty patients with hypertension were divided into control group (n = 30) and olmesartan group ( n = 30). The non - invasive ultrasonic examination was operated to observe the carotid artery intima - media thickness(IMT) and brachial artery flowmediatede vasodilatation (FMD) function of pre - and post - treatment. In addition, the concentration of blood endothelin- 1 (ET - 1 ) and nitric oxide (NO) were measured through radio - immunity and nitrate reductase method, respectively. Results The blood pressure has no difference from olmesartan group with control group after pre - and post - treatment. After therapy with olmesartan for half a year, IMT and ET - 1 levels decrease significantly ( P 〈 0. 05). The flowmediated vasodilatation (FMD) and NO levels increase significantly (P 〈 0. 05 ). Compared with control group, the level of ET - 1 and IMT significantly decreased ( both P 〈 0. 05 ) and FMD and NO significantly increased (both P 〈 0. 05 ). Conclusion Olmesartan had anti - atherosclerotic role through improving the endothelial function, which is in - dependent of its lowering blood pressure.
