摘 要:目的通过HSS(the Hospital for Special Surgery)评定膝关节功能、Fugl-Meyer量表评定平衡功能,判断系统康复治疗创伤后膝关节功能障碍(Post traumatic knee joint dysfunction,PKJD)患者后膝关节的临床效果。方法采用HSS与Fugl-Meyer量表评定36例PKJD患者系统康复治疗前、治疗10次后、治疗结束后膝关节功能、平衡功能及膝关节的临床效果。结果治疗结束后Fugl-Meyer评分与HSS评分较治疗前、治疗10次后明显下降(均P<0.01);治疗10次后两评分较治疗前亦明显下降(P<0.05、P<0.01);治疗前、治疗10次后及治疗结束后Fugl-Meyer评分与HSS评分之间呈正相关(r治疗前=0.8536,r治疗10次后=0.6897,r结束治疗后=0.8462),均P<0.01;治疗前、治疗10次后及治疗结束后膝关节的临床优良率差异有统计学意义(P<0.01)。结论系统康复治疗可改善PKJD患者膝关节功能及平衡功能,两功能间呈正相关,且治疗的次数越多,膝关节的临床效果越好。Objective To evaluate the clinical effects of systematic rehabilitation on the post-traumatic knee joint dysfunction(PKJD).Methods The special surgery scale(HSS) and Fugl-Meyer scale were used to judge the knee joint function,balance function and the clinical effects in 36 cases undergoing PKJD at baseline,10 times and the end of treatment respectively.Results Fugl-Meyer score and HSS score were significantly decrease at the end of treatment compared with the score at baseline and 10 times(P0.01),the two scale score were also decreased at 10 times as compared with baseline(0.01P0.05,P0.01),there was a positive correlation between the Fugl-Meyer score and HSS score(r=0.854,r=0.690,r=0.846,P0.01),and the significant clinical effects were seen on the knee joint function at the end of the treatment(P0.01).Conclusion Systematic rehabilitation therapy can improve the PKJD knee function and balance function,and there is a significant positive correlation between these two functions.The better clinical effect can be achieved be increasing the number of treatment.
