超声引导经皮穿刺肺活检的临床应用

摘　　要：目的评价超声引导经皮穿刺肺活检的诊断正确性和安全性。方法应用16G穿刺活检针对160例肺部病变患者行超声引导下活检,标本送组织病理检查。随访6个月获得最终诊断。结果 160例患者中诊断率为89.4%(143/160);其中恶性病变42.5%(68/160),良性病变占46.9%(75/160)。恶性肿瘤确诊率97.1%(66/68),良性病变确诊率77.3%(58/75)。穿刺术后并发症为气胸3.75%(9/160)。咯血2.5%(4/160)。结论超声引导经皮肺穿刺活检术是一种安全有效的诊断技术,具有定位准确、确诊率高、并发症少的优点。Objective To evaluate the safety and accuracy of ultrasound-guided percutaneous cutting needle biopsy of lung in inpatients with pulmonary lesions.Methods Guided by ultrasound,the percutaneous lung biopsy were performed in 160 cases of pulmonary lesions with 16-gauge needle.All of these cutting specimens were sent for histopathological examination.And the results were compared with the final diagnosis which was based on 6 months clinical follow-up.Results The histological adequate specimen was obtained in 89.4%（143/160）,sixty and eight lesions（42.5%,68/160） were malignant,and 75 lesions（46.9%,75/160） were benign.Malignant tumor confirmative diagnosis rate 97.1%（66/68） and benign pathological change confirmative diagnosis rate 77.3%（58/75）.The complications after biopsy operation were pneumothorax（9/160） and hemoptysis（4/160）.Conclusion Percutaneous lung puncture biopsy guided by ultrasound is a safe and effective diagnostic techniques with accurate positioning,high rate of diagnosis and less complications.
