摘　　要：目的 探讨外固定架联合封闭负压吸引治疗伴有大面积撕脱伤的儿童下肢骨折的效果.方法 8例伴有大面积撕脱伤的下肢骨折患儿行外固定架治疗后将撕脱的皮肤制成网状中厚皮片回植于撕脱创面,植皮区表面再行封闭负压吸引治疗,术后随访6～12个月.结果 8例患儿均未出现骨和软组织感染,骨折固定稳定,回植皮片绝大部分成活.结论 外固定架联和封闭负压吸引治疗伴有大面积撕脱伤的儿童下肢骨折具有操作简单、创伤小、固定可靠、免除换药、植皮成活率高、便于术后管理的优点.Objective To explore the effects of external fixation combined with vacuum sealing drainage （VSD） in thc treatment of lower limb fracture complicated with large-scale skin avulsion in children. Methods Eight patients with lower limb fracture and complicated with large-scale skin avulsion were enrolled in this study. The fractures were fixed using external fixators; then, the autogenous mesh grafts which were made from avulsed skin were planted on the wounds. The VSD was applied on the patients. A -200 ～-300 kPa pressure was exerted to the wound, and the VSD was used for 7 days. Results Patients were followed up for 6-12 months. No complication like bone and soft tissue infections was noted on all cases. The bone fractures were firmly fixed, and skin grafts survived in 7 cases. Conclusions The external fixation combined with VSD is simple, minimally invasion and effective in the treatment of lower limb fracture complicated with large-scale skin avulsion in children.
