摘 要:目的:探讨脂代谢异常与子痫前期发生、发展的关系。方法:测定115例子痫前期患者(其中轻度子痫前期56例,重度子痫前期59例)与50例正常孕妇血浆中总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白(Apo-A、Apo-B)、Lp(a)水平与血清游离脂肪酸(FFA)水平,并进行对比研究;测定子痫前期患者24h尿蛋白定量值。结果:轻度子痫前期组(轻度PE组)和重度子痫前期组(重度PE组)中的TC、TG、LDL、Apo-B、Lp(a)、FFA水平随着病情的进展逐渐升高,重度子痫前期组中TC、TG、Apo-B与对照组相比差异均有统计学意义(P<0.05),轻度PE组和重度PE组中的LDL、Lp(a)、FFA水平与对照组相比差异有统计学意义(P<0.05),轻度PE组和重度PE组中的HDL、ApoA1水平则随着病情进展逐渐下降,重度PE组中的HDL、ApoA1水平与对照组相比差异有统计学意义(P<0.05)。结论:子痫前期患者体内脂质代谢发生明显异常,并伴有氧化应激增强和毒性产物增加,造成血管内皮细胞损伤。脂代谢异常与该病的发生、发展密切相关。Objective:To explore the relationship between abnormal lipid metabolism and occurrence,development of preeclampsia.Methods:The levels of total cholesterol (TC),triglyceride (TG),high density lipoprotein (HDL),low density lipoprotein (LDL),apo-A,apo-B and Lp(a) in plasma and level of free fatty acid in serum in 115 cases with preeclampsia and 50 normal pregnant women were detected and compared;24 hours urinary protein excretion was detected in cases with preeclampsia.Results:The levels of TC,TG,LDL,Apo-B,Lp(a) and FFA in mild preeclampsia group and severe preeclampsia group increased gradually with the aggravation of disease;there was significant difference in levels of TC,TG and Apo-B between severe preeclampsia group and control group (P0.05).There was significant difference in levels of LDL,Lp(a) and FFA between mild preeclampsia group,severe preeclampsia group and control group (P0.05).The levels of HDL and ApoA1 in mild preeclampsia group and severe preeclampsia group decreased gradually with the aggravation of disease;there was significant difference in levels of HDL and ApoA1 between severe preeclampsia group and control group (P0.05).Conclusion:Obvious abnormality of lipid metabolism exists in patients with preeclampsia,combined with enhancement of oxidative stress and increase of toxic products,which may harm the vascular endothelial cells;abnormal lipid metabolism is related to occurrence and development of preeclampsia closely.
