Gamma钉内固定治疗股骨粗隆间骨折临床疗效

摘　　要：目的:分析Gamma钉内固定治疗股骨粗隆间骨折的临床疗效。方法:2004年1月至2009年7月,我院采用Gamma钉及动力髋螺钉(DHS)内固定两种方法治疗294例股骨粗隆间骨折患者。采用回顾性方法,分析临床资料及疗效。其中Gamma钉内固定治疗(Gamma钉组)180例,DHS内固定治疗(DHS组)114例,对手术过程、住院时间、骨折愈合时间、下地负重时间、术后功能恢复及并发症发生率进行对比进行统计学分析,结果:两组病例均获随访,随访时间8-62个月,平均37个月。手术过程中切口长度、手术时间、出血量、输血量比较差异有统计学意义(P<0.01),平均住院时间两组比较差异无统计学意义(P>0.05)。骨折愈合时间比较,差异无统计学意义(P>0.05);负重时间、髋关节功能恢复及并发症比较,差异有统计学意义(P<0.05)。结论:采用Gamma钉治疗股骨粗隆间骨折术中创伤相对较少、负重时间较早,髋关节功能恢复较好,并发症率较低,是治疗粗隆间骨折的较好方法。Objective： To investigate the therapeutic effect on femoral intertrochanteric fracture for Gamma nail.Method：We retrospectively reviewed the clinical and radiological records of 294 patients with femoral intertrochanteric fracture for Gamma nail（180 cases） and DHS（114 cases） from January 2004 to July 2009.The operation process,the average length of stay,the union of fracture and weight loading time,the functional evaluation of hip joint and the incidence of complications were compared and the curative effects were analyzed.Result：All the patients were followed-up for 37 months in average（from 8 months to 62 months）.Between the two groups,the incision length,operation time,blood loss,blood transfusion volume,weight loading time,the functional evaluation of hip joint and the incidence of complications were statistical significance（P0.05）.The union of fracture time and the average length of stay,were no statistically significant（P0.05）.Conclusion：The internal fixation of Gamma nail is a good choice for treating femoral intertrochanteric fracture.The wound is smaller,the weight loading time is earlier,the recovery of hip joint function is better,the incidence of complications is less than the DHS group.
