12例定制型人工关节置换治疗股骨远端肿瘤

摘　　要：目的股骨远端是恶性骨肿瘤的好发部位,传统治疗以截肢为主,对患者的身心造成极大的伤害。文中介绍定制型人工关节置换对股骨远端肿瘤进行保肢治疗的方法和疗效。方法选取2003年8月至2008年10月间12例进行定制型人工关节置换保肢治疗股骨远端肿瘤患者,其中骨肉瘤7例、软骨肉瘤2例、骨巨细胞瘤3例。结果12例患者获得平均35个月（10-74个月）的随访,随访时膝关节功能按Ennek ing制定的标准进行评定,优6例、良3例、差3例。2例骨肉瘤患者术后出现肿瘤肺转移死亡,其中1例关节置换术后8个月因局部复发行截肢术。结论定制型人工膝关节置换是一种较好的股骨远端肿瘤保肢方法。Objective The distal femur is a favorite spot for primary malignant bone tumors.Amputation as the mainstay of surgical treatment in the past does great harm to the patients both physically and mentally.In this study,the custom knee prosthesis was used in the limb salvage procedure for the treatment of distal femur tumors,and the techniques and clinical effect of this operation was explored.Methods From August 2003 to October 2008,12 cases of distal femur tumors were treated by custom knee prosthesis,including 7 cases of osteosarcoma,2 cases of chondrosarcoma,and 3 cases of giant cell tumors.All the cases were followed up for 10-74 months.Results Based on the Enneking evaluating system,the function of the knee joint was found to be excellent in 6 cases,good in 3,and poor in 3.Two patients with osteosarcoma died of pulmonary metastasis,of whom 1 had received amputation due to local recurrence 8 months after the knee replacement.Conclusion Total knee replacement with custom prosthesis was one of the optional methods in the limb salvage procedure for distal femur tumors.
