缬沙坦和缬沙坦联合吲达帕胺对治疗高血压病的临床观察

摘　　要：目的观察缬沙坦的降血压作用，不良反应，对代谢的影响及与吲达帕胺联用的疗效。方法选取原发高血压病患者126例，随机分为单药组（87例，缬沙坦80mg，每日1次）与联合组（39例，缬沙坦80mg／d＋吲达帕胺2．5mg／d）。由固定医生每周测量坐位血压，并于治疗前和降压治疗8周后测动态血压、心率，治疗前及治疗4周和8周后检查空腹血钾、血糖、血脂。结果治疗2周时，单药组患者血压达标率为4138％，联合组患者血压达标率为79．49％．两组血压迭标率比较有显著差异性（P〈0．01）。治疗后4周、8周后，单药组总有效率分别迭816％、874％，联合组总有效率分别选82．7％、94．8％。两组降压的谷／峰比值在60％以上，联合组24h尿白蛋白的改善有明显的统计学意义（P〈0．01），对血钾、糖代谢和脂代谢无明显影响。仅个别病人出现轻微的不良反应。结论两组都能有效地平稳降低高血压，不良反应少，对血钾、糖代谢和脂代谢无明显影响。而联合组优于单用组，能早期、平稳达到血压控制目标，能明显改善24h尿白蛋白，时保护心脏、脑、肾脏有更好效果，二者联用有良好的协同作用。Objective To study the effect of Valsartan and the combination of Valsartan and Indapamide on blood pressure and the side effect. Methods： 126 Hypertensive patients were recruited and divided into group Single medicine group （n=87,Valsartan 80mg/day） and Joint group （n= 39,Valsartan 80mg/day with inpadamide 2.5mg/day）. Sitting blood pressm~ was measured by the same doctors every a week. Before and after 8 week treatment, ambulatory blood pressure monitoring （ABPM） was performed. Fasting blood sugar,blood lipid,blood potassium were examined before and after 4 week treatment. Results Single medicine group and Joint group compliance rate of patients with blood pressure 41.38% ,79.49% after 2 week treatment,Standard rate of blood pressure compared the two groups was significantly different （P〈0.01）.After 4,8 week treatment by Single medicine group the total responsive rate was 81.6%,87,4%,Joint group the total responsive rate was 82.7%,94.8%.The trough to peak ratios of blood pressure was above 60%.Joint group 24-hour urinary albumin was significantly improved significantly （P〈0.01）.Blood sugar, blood lipid and blood potassium were not significantly changed. Side effects were few, slight and tolerable. Conclusion Two groups can effectively lower blood pressure stable,fewer adverse reactions,without significant change in blood sugar, Blood lipid and blood potassium. Combined group was better than single group, to early and steady to achieve blood pressure goal, can significantly improve the 24-hour urinary albumin, the protection of the heart,brain, kidneys have better results,The effects of two together are additive
