阿德福韦酯联合复方鳖甲软肝片治疗乙型肝炎肝纤维化临床观察

摘　　要：目的:观察阿德福韦酯联合复方鳖甲软肝片治疗乙型病毒性肝炎肝纤维化的疗效。方法:将82例乙型病毒性肝炎肝纤维化患者随机分为治疗组和对照组。治疗组患者用阿德福韦酯和复方鳖甲软肝片口服,对照组患者采用阿德福韦酯治疗,疗程1年。观察并比较两组患者治疗前后的症状、体征和肝功能、肝纤维化指标、B超指标及HBVDNA定量等变化。结果:治疗后治疗组总有效率为90.4%,对照组总有效率为77.5%,治疗组疗效高于对照组;治疗组患者在治疗后AST、TBil、HA等较治疗前有明显改善,且显著优于对照组;患者治疗后门静脉和脾静脉宽度、脾脏厚度明显变小,与对照组比较差异显著;治疗后HBVDNA定量变化,两组相比较差异无显著性意义。结论:阿德福韦酯联合复方鳖甲软肝片治疗,能够通过抑制病毒和抗纤维化,明显改善乙型病毒性肝炎肝纤维化患者的病情及预后。Objective：Clinical research of adefovir dipivoxil tablets combined with Fufangbiejiaruangan tablets on treating hepatic fibrosisn following chronic hepatitis B.Methods：Eighty-two cases of hepatic fibrosis following chronic hepatitis B were divided randomly into treatment group and contrast group.The treatment group was treated by adefovir dipivoxil tablets combined with Fufangbiejiaruangan tablets orally.The contrast group was treated by adefovir dipivoxil tablets.After 12 months of therapy,indices including liver histology,liver function,liver fibrosis,B ultrasonic,quantification of HBV DNA were tested and compared.Results：After treatment,the total effective rate of the treatment group was 90.4%.The total effective rate of the contrast group was 77.5%.The clinical effect of treatment group was significantly better than that of contrast group.After treatment,the AST,TBil,HA levels of the treatment group were improved obviously compared with those before treatment.The clinical effect of treatment group was significantly better than that of contrast group.The diameters of hepatic portal veins and splenic veins,the thickness of spleen were significantly less than those of contrast group.After treatment,the differences in quantitative detection of serum HBV DNA between the two groups did not have any statistic significance.Conclusion：The treatment of adefovir dipivoxil tablets combined with Fufangbiejiaruangan tablets has greatly improved patients condition as well as prognosis by antivirus and anti-fibrosis.
