四肢严重创伤联用外支架与封闭负压引流早期观察

摘　　要：目的：观察四肢严重创伤联合使用骨折外固定支架（以下简称外支架）与封闭负压引流技术（Vacuum sealingdrainage,VSD）的临床疗效。方法：本组病例均为四肢开放性粉碎性骨折合并皮肤软组织缺失、坏死或感染,给予外支架固定骨折同时使用VSD处理创面。结果：大部分患者持续负压引流5~7d后,创面再经植皮或直接缝合愈合;骨折无分离移位;外支架无松动移位断裂。结论：联用外支架与VSD处理四肢严重创伤稳定骨折迅速,封闭引流创面安全有效,并发症少。Objective To observe the clinical effect of treating the limbs＇ severe trauma with the fracture external fixator（hereinafter referred to as outer support）and vacuum assisted closure（VSD）.Method Cases were open and comminuted fractures of the limbs with skin and soft tissue missing,necrosis or infection,their fracture was fixed by outer support and their wound was treated by VSD.Results The continuous negative pressure drainage in most patients for 5~7 days,their wound cicatrized after skin grafting or direct suture;fracture without separation or displacement;outer support without loosening,displacement or breaking.Conclusion To treat the limbs＇ severe trauma with the outer support and VSD can fix fractures firmly and rapidly,at the same time,to close and drainage wound is safe and effective,it has fewer complications.
