胃癌淋巴结转移灶对顺铂体外化疗的药敏性及多种耐药相关因子的表达

摘　　要：目的:探讨胃癌淋巴结转移灶对顺铂(DDP)的体外化疗药敏性及其组织中多种耐药因子的表达。方法:对54例胃癌组织及相应转移淋巴结的肿瘤细胞进行MTT法原代细胞培养体外化疗药敏性实验,检测肿瘤细胞对DDP的体外化疗药敏性;采用免疫组织化学方法检测原发灶、淋巴结转移灶组织中P-gp、GST-π、P53、Survivin、Bcl-2和Bax6种多药耐药相关因子的表达情况。结果:CDDP对淋巴结转移灶肿瘤细胞的抑制率为(26.10±10.03)%,与原发灶的(27.04±8.37)%比较,差异无统计学意义(t=0.530,P=0.597)。胃癌原发灶GST-π表达水平越高药物抑制率越低(β=-2.937,P=0.010);转移淋巴结中P53表达水平越高药物抑制率也越高(β=2.015,P=0.026),Survivin、Bcl-2表达水平越高药物抑制率越低(β分别为-2.652和-3.466,P=0.046和0.002)。P-gp、GST-π、Bcl-2和Bax在淋巴结转移灶中的表达程度高于原发灶(Z=2.414,P=0.016;Z=2.218,P=0.026;Z=2.096,P=0.036;Z=3.175,P=0.001);胃癌原发灶、淋巴结转移灶中P-gp、P53、Bcl-2及Bax的表达均呈正相关(rs=0.342,P=0.011;rs=0.712,P<0.001;rs=0.455,P=0.001;rs=0.311,P=0.022)。结论:影响胃癌原发灶和淋巴结转移灶中DDP药敏性的耐药相关因子不同。Aim：To investigate the relationship between expressions of multidurg resistance（MDR） associated factors and chemosensitivity to cisplatin（DDP） in lymph node metastasis（LNMs） tissue of gastric carcinoma.Methods：The chemosensitivity of tumor cells to DDP was measured by MTT assay,and the expressions of P-gp,GST-π,P53,Survivin,Bcl-2 and Bax were detected immunohistochemically in 54 paired primary tumor（PT） and LNMs of gastric carcinoma.Then the relationship between chemosensitivity in vitro and expression of MDR factors was investigated.Results：There was no significant difference between the inhibition rates of DDP to LNMs or PT（t = 0.530,P = 0.597）.There was negative correlation between inhibition rates and expression of GST-π in PT（β =-2.937,P = 0.010） ;in LNMs,Survivin,Bcl-2 had negative correlation with inhibition rates（β =-2.652,-3.466,P = 0.046,P = 0.002）,and positive correlation was found between P53 and inhibition rates（β = 2.015,P = 0.026）.The expressions of P-gp,GST-π,Bcl-2 and Bax were higher in LNMs than in PT（Z = 2.414,P = 0.016;Z = 2.218,P = 0.026;Z = 2.096,P = 0.036;Z = 3.175,P = 0.001） ;and positive correlation of P-gp,P53,Bcl-2 and Bax was found between PT and LNMs（rs = 0.342,P = 0.011;rs = 0.712,P 0.001;rs = 0.455,P = 0.001;rs = 0.311,P = 0.022）.Conclusion：The main MDR factors that affected chemosensitivity to DDP ware different between PT and LNMs in gastric carcinoma.
