某省医疗行业违法现状分析

摘　　要：目的:通过卫生监督检查,掌握医疗机构执业的基本情况,为制定进一步的监督措施,从而更好地保护人民群众身体健康和生命安全提供科学的决策依据。方法:按照卫生部统一编制的调查表,对某省所有取得医疗机构执业许可证的医疗机构以及经举报和日常监督发现的无证行医机构进行调查。结果:共调查了33151家医疗机构,查处的违法违规中聘用非卫生技术人员占17.53%,超范围执业占13.86%,出租承包科室占1.20%。营利性医疗机构聘用非卫生技术人员的比例为33.82%,高于非营利性医疗机构的19.96%(χ2=514.327,P<0.001);营利性医疗机构出租承包科室的比例为0.91%,低于非营利性医疗机构的2.95%(χ2=120.283,P<0.001);营利性医疗机构超范围执业的比例为20.78%,高于非营利性医疗机构的22.44%(χ2=8.679,P=0.003)。共查处12195家无证行医机构,其所在场所中45.66%分布在农村,29.91%分布在城市结合部,24.43%分布在城市社区,3者在分布构成上比较,差异有统计学意义(χ2=369.464,P<0.001)。结论:某省医疗服务市场存在违法违规的现象较为严重,应加强对医疗市场的监督力度。Aim：Linking with the fact in a province,this paper systematically analyzed the legal medical practice and illegal or out of line act of the practitioners in medical service market,so as to provide a scientific decision gist for normalizing medical service market and enforcing government regulation.Methods：In accordance with the questionnaire authorized by the Ministry of Health,selecting the status of the illegal medical practice in Henan Province,this paper made aggregation,description and conclusion.Results：There were 33 151 medical institutions that had been investigated in total.In the actuality of the illegal or out of line medical practice,17.53% of the medical institutions employed staffs who had no certificate and background of medicine,13.86% of them conducted practice beyond their bounds allowed,and 1.20% of them leased subsidiaries.The percentage of profit-making medical institutions＇hiring non-professionals was 33.82%,significantly higher than 19.96% of non-profit-making medical institutions（χ2 = 514.327,P 0.001）.The percentage of those profitmaking medical institutions＇ leasing their departments was 0.91%,significantly lower than that of those non-profit-making ones（2.95%）（χ2 = 120.283,P 0.001）.22.44% of those non-profit-making institutions had practice beyond the authorized bounds while 20.78% of those profit-making ones had similar practice（χ2 = 8.679,P = 0.003）.There were 12 195 medical setups without certificate at all.Distribution of illegal medical setups：45.66% of them chose countryside,29.91% chose conjunction area,24.43% chose town communities,and there was significant difference among the three groups in the distribution（χ2 = 369.464,P 0.001）.Conclusion：Illegal or out of line medical practice is very common in the province.So government should strengthen the inspection and management of medical institutions and monitor medical market responsibly.
