新型猪心包源硬脑膜修补材料的实验研究

摘  要:目的 评价新型猪心包源人工硬脑膜材料用于修补缺损硬脑膜的可行性及安全性.方法 36只新西兰兔按完全随机法分3组:A组(实验组,猪心包源人工硬脑膜修补材料组)、B组(自体颅骨骨膜组)和C组[膨体聚四氟乙烯(e-PTFE)硬脑膜组],每组12只.构建硬脑膜缺损动物模型,分别应用猪心包源人工硬脑膜修补材料、自体颅骨骨膜、e-PTFE硬脑膜行硬脑膜缺损修补.每组动物分别于术后30、90、180d各处死4只,并于术前及处死前采集兔静脉血行白细胞、淋巴细胞计数.处死后于修补材料部位采集标本行组织学检查,对比观察分析植入材料局部细胞浸润及局部蛛网膜、硬脑膜下腔和脑皮质情况.结果 3组动物术后无脑脊液漏发生及并发症出现.A、B、C组动物术前白细胞数[(6.94±0.39)、(6.94±0.60)、(6.94±0.41)个/mm2]和淋巴细胞计数[(4.77±0.41)、(4.61±0.57)、(4.78±0.39)个/mm2]与术后白细胞数[(7.01±0.49)、(6.75±0.26)、(7.20±0.49)个/mm2]和淋巴细胞数[(4.60±0.55)、(4.54±0.26)、(4.46±0.76)个/mm2]差异均无统计学意义(P〉0.05).3组植人材料与脑皮层粘连等不良反应差异均无统计学意义(P〉0.05).病理观察3组的硬脑膜修补部位无变性、包裹、钙化,局部炎性细胞浸润差异也无统计学意义(P〉0.05).结论 新型猪心包源人工硬脑膜是一种安全、可行的硬脑膜修补材料.Objective To evaluate the safety and efficacy of a dural graft derived from porcine pericardium in duraplasty. Methods Thirty-six New Zealand rabbits were randomly divided into 3 groups:Group A:new dural substitute derived from porcine pericardium;Group B:autologous periosteum from cranial bone;Group C:e-PTFE dural graft. Each group consisted of 12 rabbits. All the animals underwent right parietal craniectomy behind the coronal suture and beside the midline to expose the dura. The dura was cut off and replaced with new dural substitute graft derived from porcine pericardium,autologous periosteum from cranial bone and e-PTFE dural graft,respectively. Four animals in each group were sacrificed on days 30,90 and 180,respectively. Before the operation and sacrifice,the peripheral vein blood was collected and counted for WBC and lymphocytes. The meningocerebral adhesion was observed. The slice was stained with hematoxylin-eosin and histologically examined. The local inflammatory cell infiltration around the dural substitute was observed and analyzed statistically. The local arachnoid,subdural space and cortical was observed. Results None of the animals showed leakage of cerebrospinal fluid and postoperative complications. The counts of WBC and lymphocytes in A,B and C groups before operation[(6.94±0.39),(6.94±0.60) and (6.94t0.41)/mm2;(4.77±0.41),(4. 61±0.57) and (4.78±0.39)/mm2]did not differ from the findings after operation[(7.01±0.49),(6.75±0.26) and (7.20±0.49)/mm2;(4.60±0.55),(4.54±0.26)and (4.46 ± 0.76)/mm2](P〉0.05). Three groups using various grafts showed comparable degree of meningocerebral adhesion. Microscopic examination demonstrated tissue repair on the site of implantation ineach group. Degeneration,calcification,encapsulation and local inflammatory infiltration were not statistical different among 3 groups (P〉0.05).
