摘 要:目的:初步建立纳米细菌(nanobacteria,NB)大鼠肾结石模型,探讨NB与肾结石形成的相关性和成石机制。方法:应用人肾结石患者血清培养NB。SD雄性大鼠30只随机分成NB诱石组(A组,鼠尾静脉注射NB)、NB+四环素组(B组,鼠尾静脉注射NB和胃饲四环素)、空白对照组(C组,鼠尾静脉注射生理盐水),每组各10只。8周后处死大鼠,镜下观察肾组织晶体分布并计数。检测各组大鼠24h尿N-乙酰-β-D-氨基葡萄糖苷酶(N-acety1-β-D-glucosaminidase,NAG)、乳酸脱氢酶(lactate dehydrogenase,LDH)浓度。结果:A组和B组大鼠肾脏肾小管内产生含钙结晶,Von kossa染色阳性,主要分布于远曲小管和近曲小管,C组大鼠肾组织未见晶体,3组间差异有统计学意义(P=0.033)。B组晶体少于A组,有减少趋势。24h尿NAG和LDH浓度在3组间差异有统计学意义(P<0.001)。结论:NB通过肾小管上皮细胞损伤机制诱导大鼠肾脏肾小管内产生含钙结晶,参与肾结石形成。四环素通过抗NB机制,可能有抑制结晶形成和肾结石形成作用。Objective:To establish nephrolithiasis rat model induced by nanobacteria and to determine the relationship of nanobacteria with nephrolithiasis.Methods:Thirty adult male SD rats were rando-mized into 3 groups(n=10 each):group A,given an intravenous injection of nanobacteria;group B,given an intravenous injection of nanobacteria and taken tetracycline;group C,as normal control.Eight weeks later,all rats were sacrificed and the kidneys were examined for pathology and the 24-h urine N-acetyl-β-D-aminoglycoside enzyme(NAG) and lactate dehydrogenase(LDH) were detected.Results:Histopathological studies revealed nanobacteria induced renal tubular crystallization,which was significantly different among the 3 groups(P=0.033).The crystallization was higher in group A than that in group B.The urinary NAG and LDH excretion were significantly different among the 3 groups(P0.001).Conclusion:Nanobacteria may be involved in the pathogenesis of nephrolithiasis.Tetracycline may inhibit stone formation.
