摘 要:目的探讨肿瘤坏死因子(TNF-α)在系统性红斑狼疮(SLE)中的变化及其与感染发生的关系。方法收集22例非感染组SLE、20例合并肺部感染SLE病例及20例正常对照组血清,用试剂盒检测TNF-α浓度,感染组病例治疗后再次采血复查。结果各组TNF-α浓度中位数为正常对照组23.35,非感染组36.63,感染组治疗前后分别是99.21和51.26。非感染组高于正常对照组,感染组高于非感染组,感染组经治疗后TNF-α水平明显降低,差异均有显著性(P<0.001)。结论 TNF-α在SLE的发病过程中具有重要作用,动态观察其水平变化在感染的诊断和预测及考核治疗效果有一定的价值。【Objective】To determine the expression of tumor necrosis factor-alpha(TNF-alpha) in systemic lupus erythematosus(SLE) and its relationship with infection.【Methods】Levels of TNF-alpha in serum samples from 64 individuals(22 with SLE without infection,22 with SLE and pulmonary infection and 20 healthy controls) were examined with ELISA.Levels of TNF-alpha were tested twice in SLE-infected patients before and after antiinfection treatment.【Results】The median TNF-alpha levels were 23.35 in healthy controls,36.63 in SLE noninfected patients,99.21 in SLE-infected patients before treatment and 51.26 after treatment.Levels of TNFalpha were higher in SLE-infected patients than in healthy controls.They were higher in SLE-infected patients than SLE-noninfected patients.Levels of TNF-alpha were significantly decreased in SLE-infected patients after antiinfection treatment.The differences were statistically significant(P 0.001).【Conclusions】Level of TNF-alpha is helpful to diagnose infection in SLE and to assess treatment effect.
