摘  要:目的观察琥珀酰明胶(MFG)和脲联明胶对盲肠结扎穿孔(CLP)所致脓毒症大鼠肺毛细血管漏及炎性反应的影响。方法 96只雄性SD大鼠,随机分为八组,每组12只,CLP+聚明胶肽组(聚明胶肽分别7.5、15和30 ml/kg,为Ⅰ、Ⅱ、Ⅲ组)和CLP+MFG组(MFG分别7.5、15和30 ml/kg,为Ⅳ、Ⅴ、Ⅵ组);CLP组(NS 30 ml/kg,Ⅶ组);假手术组(NS 30 ml/kg,Ⅷ组)。各组术后4 h按要求静脉输注相应的液体,输注速率为0.2 ml/min。6 h后处死大鼠,取肺组织,分别测量肺毛细血管通透性、含水量、肿瘤坏死因子α(TNFα-)表达、核因子κB(NFκ-B)活性和超微结构改变。结果脲联明胶(15 ml/kg和30 ml/kg)和MFG(30 ml/kg)均可显著减少肺毛细血管漏,但MFG降低作用更强(P<0.05),两种明胶均可改善肺超微结构,但均不能降低TNFα-的表达和NFκ-B的活性。结论聚明胶肽与MFG均可减轻肺毛细血管漏,改善肺组织的超微结构改变,但与炎症反应无关。Objective To investigate the effects of succinylated gelatin and urea-linked gelatin on pulmonary capillary permeability and inflammation in rat sepsis model by cecal ligation and puncture(CLP).Methods Ninety-six male SD rats were randomly divided into 8 groups with 12 rats each.Group Ⅰ,Ⅱ,Ⅲ received CLP+polygeline 7.5,15 and 30 ml/kg,respectively.Group Ⅳ,Ⅴ,Ⅵ received CLP+MFG 7.5,15 and 30 ml/kg,respectively.Group Ⅶ received CLP and NS 30 ml/kg.Group Ⅷ received sham operation and normal saline(NS) 30 ml/kg.The animals received transfusion 4 hours after CLP.The lung tissues were acquired 6 hours after CLP for the determinations of pulmonary capillary permeability,water content,TNF-α level,NF-κB activity and ultrastructure.Results Urea-linked gelatin(15 ml/kg and 30 ml/kg) and succinylated gelatin(30 ml/kg) significantly decreased pulmonary permeability and improved ultrastructure changes,without depression on TNF-α and NF-κB.Conclusion Both succinylated and urea-linked gelatins can reduce CLP-induced pulmonary capillary leak and improve ultrastructure without decrease of inflammation.
