70岁以上老年患者结直肠癌肝转移同步手术切除的临床分析

摘　　要：目的探讨70岁以上患者结直肠癌肝转移同步手术切除的可行性。方法收集结直肠癌肝转移老年患者98例（60～84岁，平均70岁）行结直肠癌肝转移同步手术切除，其中，39例〉70岁患者为观察组（71—84岁，平均76．9岁），59例≤70岁为对照组（60～70岁，平均65．5岁）。回顾性分析围手术期病死率、平均住院时间、术后并发症、中位生存时间。结果围手术期均无死亡病例，术后并发症33例（33．7％，33／98），其中观察组13例（33．3％，13／39），对照组20例（33．9％，20／59）；平均住院时间15d，其中观察组16d，对照组14d；生存时间8～81月，中位生存时间40．5月，其中观察组生存时间8～74月，中位生存时间39．4月，对照组生存时间11—81月，中位生存时间41．2月。结论70岁以上老年患者也可以行结直肠癌肝转移同步手术切除。Objective To investigate the feasibility and safety of combined resection of hepatic metastases and colorectal cancer for elderly patients over 70 years old. Methods 98 elderly patients with colorectal cancer and liver metastases, 39 being aged over Toc 76.9, （71-84） （observation group） and 59 aged ≤70 （65.5, 60-70） （control group）, underwent combined resection of hepatic metastases and eolorectal cancer. The perioperative mortality, postoperative complications, mean hospitalization time, and median survival time were analyzed between these 2 groups. Results There was no death during the perioperative period. Totally 33 patients had postoperative complications （33.7%, 33/98）, 13 （33.3%, 13/ 39） were in the observation group and 20 （33.9%, 20/59） in the control group without significant difference between these 2 groups. The mean hospitalization time of the 98 patients was 15 days. The mean hospitalization time of the observation group was 16 days, not significantly different from that of the control group （ 14 days, P〉0.05）. The median survival time of the 98 patients was 40.5 （8-81） months, The median survival time of the observation group was 39.4 （8-74） months, not significantly different from that of the control group [41.2 （11-81） months, P〉0.05]. Conclusion Combined resection of hepatic metastases and colorectal cancer is feasible and safe for elderly patients over 70.
