1804例麻风复杂性足底溃疡的综合治疗

摘　　要：在12个试点地区使用外科手术、石膏管型、支具、更换敷料、休息、防护鞋及改良的鞋垫等综合方法,对1804例麻风患者的2599个复杂性足底溃疡进行了治疗。结果1055例患者的1446个溃疡完全愈合(55%),院内的占70%。治愈者中有172人219个溃疡复发(16%),院外的复发数超过18%。综台治疗复杂性足底溃疡愈合率高,复发率低。减轻劳动强度、避免过度行走、加强足的自我保护知识教育、提供劳动救济、经济支持是预防溃疡复发的主要措施。A comprehensive treatment,including surgical intervention,plaster immobi- lization,supports,dressing,rest and wearing protective footwears with modified insole,was giv- en to 1804 leprosy patients with 2599 complicated plantar ulcers.The result showed that 1446 ulcers in 1055 cases healed,accounting for 55%,and the healing rate in those who live in lep- rosy hospitals was 70%.219 ulcers in 172 cases (16%) recurred later.The recurrence rate in the patients living at home was more than 18%.The authors think that the comprehensive treat- ment for the ulcers have higher healing rate and lower recurrence rate,but reduction of work- load,avoidance of distant walking,intensification of education on foot self—care and provision of financial support are essential to prevention of the ulcers.
